Laparoskopik Sleeve Gastrektomi Materyallerinde Histopatolojik Bulguların Değerlendirilmesi
Laparoskopik sleeve gastrektomi (LSG) morbid obezitede sık kullanılan bir tedavi yöntemi olup, bu spesmenlerde rutin histopatolojik incelemenin değeri tartışmalıdır. Ayrıca LSG yapılan hastalarda midedeki HP kolonizasyonunun etkisi bilinmemektedir. Bu çalışmada amacımız LSG spesmenlerindeki histopatolojik inceleme sonuçlarını değerlendirmek ve HP prevalansını belirlemektir. Bu çalışma hastanemizde 2014-2018 yılları arasında morbid obezite nedenli LSG yapılan hastaları içeren retrospektif bir çalışmadır. Yaş, cinsiyet, cerrahi spesmenlerdeki histopatolojik bulgular ve HP infeksiyon prevalansı değerlendirildi. Bu çalışmada 110 hastanın (90 kadın, 20 erkek; ortalama yaş 38 ± 11.9 yıl) gastrik spesmeni incelendi. Cerrahi spesmende sık gözlenen histopatolojik bulgular kronik inaktif gastrit (%57.3, n=63), kronik aktif gastrit ( %34.5, n= 38) intestinal metaplazi (%6.4, n=7), minimal atrofi (%12.7, n=14), fundik gland polip (%1.8,n=2), nöroendokrin hücre hiperplazisi (%1.8’inde, n=2), pariyetal hücre hipertrofisidir (%0.9 , n=1). Kırk sekiz hastada (%43.6) HP pozitifti. Gastrit ve atrofi ile HP infeksiyonu arasında istatiksel olarak anlamlı korelasyon saptandı p<0.001). LSG yapılan olguların çoğunda histopatolojik değişiklikler saptanmıştır. H.P infeksiyonu, atrofi ve intestinal metaplazinin klinik önemi nedeniyle LSG spesmenlerinin histopatolojik değerlendirmesinin gerekli olduğu sonucuna varılmıştır.
Evaluation of Histopathological Findings in Patients Undergoing Laparoscopic Sleeve Gastrectomy Specimens
Laparoscopic sleeve gastrectomy (LSG) is a common surgical therapeuti coption in patients with morbid obesity that debate about the value of routine histopathologi cal examination of LSG. Also the effect of Helicobacterpylori colonization of the stomach of patient sunder going LSG is unclear. Ouraim is to analyze there sults of histopathological examination of LSG specimensan didentify the prevalence of Helicobacterpylori. This study is a retrospective study of the patients who underwent LSG for the treatment of morbid obesity in our hospital between 2014-2018. Sex, age, histopathologi calfindings in the surgical specimens and prevalenceof Helicobacterpylori(HP) infection were analyzed. Gastricspecimens of a total of 110 patients( 90females, 20 males; meanage 38 ± 11.9years)were analyzed in this study.Themost common histopathological findings in the surgical specimens were chronicin active gastritis (57.3%,n=63),chronic active gastritis ( 34.5%, n= 38),intestinal metaplasia (6.4%, n=7), fundic gland polyp (1.8%,n=2), parietal cellhy pertrophia (0.9 %, n=1). Fourty eight (43.6%) patients were foundto be HP positive. There was statistically significant correlation between gastritis and HP infections. The majority of patients who underwent LSG had pathological findings in resected specimens.It is concluded that the pathological evaluation of the LSG specimens is necessary because of the clinical significance of HP infection and intestinal metaplasia.
