We performed deep dorsal vein resection of the penis ( DDVRP ) in 26 impotent men who were determined to have venous leak by pharmacocavernosometry. Postoperatively, by mean follow-up period of 18 months, six patients (23.1 %) had unaided erections, seven became responsive to intracavernous injection (ICI) of papaverine (26.9%). Total success rate was 50%. Three patients underwent placement of penile prosthesis. Ten patients did not accept any further therapy. Smoking and age were found to be significantly different between the success and failure group. Despite modest success rates with long-term follow-up venous surgery can be offered to younger nonsmoker patients.
