ADNAN MENDERES ÜNİVERSİTESİ TIP FAKÜLTESİ YENİDOĞAN YOĞUN BAKIM ÜNİTESİNDE İZLENEN ÇOK DÜŞÜK DOĞUM AĞIRLIKLI BEBEKLERİN DEĞERLENDİRİLMESİ
Amaç:Bu çalışmada ADÜ Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Yenidoğan Yoğun BakımÜnitesi'nde izlenen çok düşük doğum ağırlıklı bebeklerin mortalite ve morbidite oranlarının retrospektif olarakdeğerlendirilmesi amaçlandı.Yöntem: Ocak 2000-Aralık 2005 tarihleri arasında izlenen 1500 gram ve altında doğum ağırlığına sahip 61bebeğin verileri incelendi. Çalışmaya alınan bebeklerin sosyodemografik özellikleri ve perinatal olaylar,hastanede izlendikleri dönemde gelişen sepsis, intrakranial kanama, nekrotizan enterokolit, prematüreretinopatisi, surfaktan uygulanma oranı, kronik akciğer hastalığı, ventilatör desteği alanlarda; süresi, mortaliteoranları ve zamanları, yatış süreleri değerlendirildi. Değerler ortalama ± standart sapma, ortanca (en düşük ve enyüksek değer) olarak verildi. Sıklık ve tanımlayıcı istatistiklerin yanısıra student- testi ve Pearson korelasyonanalizi kullanıldı.Sonuçlar:Çalışmaya alınan olguların %62'si erkek, %3'ü ambigius genitale olup ortalama gestasyon yaşı29.0±2.2 hafta, doğum ağırlıkları 1187±243 gr idi. Ortalama anne yaşı 27.4± 6.2 yıldı. Olguların %57'si vaginal yolla doğmuştu, 1. dakika Apgar skoru ortalaması 5.9±2.3, 5.dakika Apgar skoruortalaması 7.7±1.9 saptandı. Olguların %46.5'i hastanemizde doğmuştu ve %37'sinde 24 saatten uzun erkenmembran rüptürü öyküsü vardı.Olguların %20.8'ine surfaktan uygulanıp, %70'i ventilatör desteği aldı ve ventilatörde kalma ortanca süresi 66saat olarak bulundu. Tüm grupta %26.4 olguda kronik akciğer hastalığı, %16'sında patent duktusarteriozus, %6.5 olguda evre 2 nekrotizan enterokolit, %9.8 olguda intrakranial kanama, %8.2 olguda evre ¡2retinopati saptand?. İzlem süresince %31.8 olguya kan kültürü ile ispatlanan sepsis tanısı konuldu, %21'indecandidatürleri üretildi. Olguların sağ kalım oranı %82 olarak saptandı. Doğum ağırlığı ve gestasyon yaşıazaldıkça mortalite oranı artış gösteriyordu.Sonuç:Sonuç olarak mortalite oranlarımızın gelişmiş ülkelere göre daha yüksek, ancak ülkemizdeki oranlarabenzer nitelikte olduğu, ünitemizdeki surfaktan uygulama oranının gelişmiş ülkelere göre daha düşük olduğu,bunun da mekanik ventilasyon ihtiyacını, ventilatörde kalma süresini ve kronik akciğer hastalığı oranını artırdığı söylenebilir
Outcome of Very low BirthWeight Infants Followed in the Neonatal Intensive Care Unit of Adnan Menderes University Faculty of Medicine
Purpose: The aim of the study was to determine the mortality and morbidity rates for infants weighing 1500 g or less who were admitted to the neonatal intensive care unit of our hospital between 2000 and 2005. Material and Methods: The charts of 61 low-birth-weight infants were evaluated retrospectively. The data consisted of patient information including socio-demographic characteristics, perinatal events, procedures such as surfactant replacement therapy, respiratory support and selected patient outcomes such as sepsis, intracranial hemorrhage, necrotizing enterocolitis, retinopathy of prematurity, chronic lung disease, mortality and morbidity rates and hospitalization times. Descriptive and frequency statistics, student-t test, Pearson correlation tests were used for statistical analysis. Results: Of 61 infants, 62% were male, 3% had ambiguous genitalia. The mean gestational age and birth weight were 29.0±2.2 (25-35) weeks and 1187±243 (580-1500) g,respectively. Fortysix and a half % of the cases had been delivered in our hospital and37%had premature and prolonged membrane rupture. The surfactant therapy rate was 20.8% and 70% of the cases had mechanical ventilation support. The median ventilation duration was 66 hours (0-960). The incidence of chronic lung disease was 26.4%. The incidence of sepsis diagnosed by blood culture was 31.8%. The survival rate was 82%. Conclusion: Our mortality rate was higher than developed countries but comparable to the rates of our country. Lower surfactant therapy rates compared with that of developed countries could lead to higher mechanical ventilation time and a higher incidence of chronic lung disease.
