A Vitamini Tedavisinin Kuru Göz Hastalığına Olan Etkisi

AMAÇ: Topikal uygulanan A vitamininin kuru göz hastalarında gözyaşı kırılma zamanı (GKZ), korneal boyanma skoru ve oküler yüzey hastalık indeksi (OYHİ) üzerine olan etkilerinin araştırılması amaçlanmıştır.GEREÇ VE YÖNTEMLER: Çalışmaya 15 hastanın 30 gözü dahil edildi. Ek oküler yüzey hastalığı olmayan, göz ameliyatı geçirmemiş ve OYHİ değeri 25’in üzerinde olan semptomatik kuru göz hastalarına 2 aylık topikal Vitamin A tedavisi (Vit-A Pos oft pomad 2x1) uygulandı. Tedavi sonrasındaki GKZ değerleri, Oxford evreleme şeması ile boyanma skorları ve OYHİ skorları tedavi öncesi değerlerle karşılaştırıldı.BULGULAR: Tedavi öncesi ortalama GKZ değeri 2,26±0,59 sn, boyanma skoru 2,53±0,74, OYHİ skoru 45±13,82 olarak bulundu. 2 aylık A vitamini tedavisi sonrasında hastaların şikayetlerinde belirgin derecede azalma olduğu öğrenildi; tedavi sonrası ortalama GKZ değeri 2,93±1,22 sn, boyanma skoru 2,06±0,79 ve OYHİ skoru 29,66±12,53 olarak bulundu. Tedavi öncesi ve sonrası değerler karşılaştırıldığında, değişimlerin istatisiksel olarak anlamlı olduğu görüldü; p değerleri GKZ için 0,027, boyanma skoru için 0,029 ve OYHİ için <0,01 olarak belirlendi. Hiçbir hastada yan etki görülmedi.SONUÇ: Vitamin A tedavisinin kuru göz hastalarının şikayetlerini azalttığı görüldü. Boyanma skorunda ve özellikle OYHİ skorunda belirgin bir azalmaya, GKZ değerinde ise anlamlı bir artışa neden olduğu görüldü. A vitamini kuru göz hastalarında semptomatik iyileşme sağlamaktadır ve kuru göz hastalarının tedavisinde alternatif bir tedavi olarak düşünülmelidir

The Effects of Vitamin A Treatment on Dry Eye Disease

OBJECTIVE: To investigate the effect of topically administered vitamin A on break up time (BUT), corneal staining score and ocular surface disease index (OSDI) in dry eye patients. MATERIALS AND METHODS: Thirty eyes of 15 patients were included in the study. Topical Vitamin A (Vit-A Pos pomad) twice a day was administered for 2 months to the semptomatic dry eye patients who have not undergone any ocular surgery, who have not got any additional ocular surface disease and who have an OSDI score of >25. BUT values, corneal staging scores with Oxford grading scale and OSDI scores after the treatment were compared with the values before the treatment. RESULTS: Mean age of the patients was 54.26±12.89. The means of BUT, corneal staining and OSDI scores before the treatment were 2.26±0.59, 2.53±0.74 and 45±13.82, respectively. After 2 mo Vitamin A treatment, patients’ complaints decreased, the means of BUT, corneal staining and OSDI scores after the treatment were found to be 2.93±1.22, 2.06±0.79 and 29.66±12.53, respectively. When compared the values before and after the treatment, differences were seen to be statically significant. P values were 0.027, 0.029 and <0.01 for BUT, corneal staining and OSDI scores, respectively. None of the patients complained about adverse effect. CONCLUSION: Vitamin A treatment was seen to reduce the complaints of dry eye patients. It was resulted in a significant decresae in especially OSDI and corneal staining scores, also a significant increase in BUT value. Vitamin A provides a symptomatic improvement in dry eye patients and should be considered as an alternative treatment in symptomatic dry eye patients.
