Kayısı çekirdeğine bağlı mekanik kalın barsak obstruksiyonu: İntestinal obstruksiyonun nadir nedeni. Olgu sunumu
Fitobezoar dünyanın her yerinde yaygın olarak karşılaşılan bir durum olup, kalin barsak obstrüksiyonunun nadir sebeblerinden biridir. Bu çalışmada kayısı çekirdeği yutulmasına bağlı bir kalın barsak obstrüksiyonu vakası takdim edilmiştir. Otuzbeş yaşında erkek hasta 15 günden beri devam eden karın ağrısı, şişkinlik ve kabızlık şikayetiyle acil polikliniğine başvurmuştur. Radyolojik tetkiklerde kitle saptanmamış, yapılan endoskopik muayenesinde çıkan kolonda endoskopun geçişine izin vermeyen darlık tespit edilerek bu bölgeden biyopsi alınmıştır. Biyopsi sonucu, fibrozis ile uyumlu gelmiştir. Laparatomide çıkan kolonda 4x5 cm kitle saptanmış ve darlık da göz önüne alınarak sağ hemikolektomi ve ileotransverstomi uygulanmıştır. Makroskopik olarak lümende çekumun çıkışında duvar kalınlaşması ve yüzeyel ülsere sebebiyet veren kayısı çekirdeği izlenmiştir. Çocuklarda, debil hastalarda ve geçirilmiş gastrointestinal sistem operasyonu anamnezi olan mekanik intestinal obstrüksiyon vakalarında, radyolüsent yabancı cisim varlığı (bezoarlar) mutlaka akılda tutulmalıdır.
Mechanical large bowel obstructıon due to apricot seed: A rare cause of intestinal obstruction. Case report
Phytobezoar which is a condition that appears everywhere in the world is very rare cause of the large bowel obstruction. In this case report, a patient with mechanical large bowel obstruction due to apricot seed swallowing is presented. A 35 year-old man admitted to our hospital, suffering from stomach pain, constipation, distension for 15 days. At radiological examinations mass wasn't been detected. At endoscopic examinations, narrowing of the lumen was observed at the ascending colon level and biopsy was performed. The result of the biopsy showed fibrosis in the wall. At laparatomic examinations a mass was found in the ascending colon and right hemicolectomi and ileotransversotomy was performed. On macroscopic examination we found an apricot seed that was causing the thickening of the wall and superficial ulcer in the mucosa. In the cases of mechanical intestinal obstructions existence of a radyolucent foreign material (bezoars) must be keep in mind, especially in the children, debile patients and the patients that have gastrointestinal system operations.
