Lomber bölgenin spinal stenozu taklit eden spontan subdural hematomu

Spontan spinal subdural hematom SSSH oldukça nadir görülen bir durumdur ve genellikle travma ya da hematolojik değişiklikler nedeniyle kısmen de spontan oluşabilir. Spinal kord basısına yol açması sonucu ciddi nörolojik hasarlara neden olabilirler. Bu yüzden hızlı ve doğru teşhis ve hemen ardından hastanın durumuna göre konservatif ya da cerrahi tedavi mortalite ve morbiditeyi etkilemesi açısından önemlidir. Burada, antikoagulan tedavi veya travma gibi SSSH'a neden olabilen bir hikayesi mevcut olmayan lomber spinal stenoz LSS hikayesi dışında bilinen bir şikayeti bulunmayan, SSSH gelişen ancak, medikal tedaviye rağmen nörolojik durumu kötüleşen ve 72 saat sonra yapılan dekompresif cerrahi neticesinde bulguları düzelen hastayı tartıştık.

Spontaneous spinal subdural hematoma of the lumbar spine mimicking spinal stenosis

Spontaneous spinal subdural hematoma SSSH is an extremely rare and usually occurs spontaneously in part due to trauma or hematologic changes. The outcomes may lead to spinal cord compression and cause serious neurological damage. Therefore, urgent and accurate diagnosis with subsequent treatment are important in order to prevent surgical mortality and morbidity. In the present case, anticoagulant therapy or history of trauma which might cause SSSH were not present, the patient had only lumbar spinal stenosis LSS . Despite medical treatment, neurological condition deteriorated progressively. Decompressive surgery after 72 hours improved the clinical condition.
