Ultrason Eşliğinde Perkütan Yolla Yapılan Karaciğer Biyopsi Uygulaması: Kocaeli Üniversitesi Girişimsel Radyoloji Deneyimi
Amaç: Ultrason (US) eşliğinde perkütan karaciğer biyopsisi, halen kronik karaciğer hastalığı ve karaciğer kitleleri yönetiminde halen altın standart yöntemdir. Çalışmamızda, Kocaeli Üniversitesi Tıp Fakültesi Girişimsel Radyoloji kliniğinde US eşliğinde son beş yılda yapılan karaciğer biyopsilerinin endikasyonları, kullanılan iğne özellikleri, hastaya ait diğer hastalıkları ve demografik verilerin değerlendirilmesi çalışmamızın amacını oluşturmaktadır. Yöntem: Ocak 2015-Mayıs 2020 arasında hastanemizde perkütan karaciğer biyopsisi yapılan hastaların endikasyonları, komplikasyonları, histolojik bulguları değerlendirilmiştir. Bulgular: Perkütan karaciğer biyopsi 264 hastada yapılmıştır. Karaciğer biyopsi endikasyonları içerisinde hepatit B(HBV) (%29,2) en sık endikasyon olup diğer endikasyonları sırasıyla malign kitleler (%22), non-alkolik yağlı karaciğer hastalığı (NAYKH) (%11,7), ilaca bağlı karaciğer hastalığı (İKH) (%11,4), Alkol (%9,5), hepatit C (HCV) (%9,5), Oto immun hepatit (%6,8)’dir. Sonuç: US eşliğinde perkütan karaciğer biyopsi, hastanede yatan ve ayaktan başvuran hastalarda başarı ile uygulanan hızlı, güvenli, etkili ve minimal invaziv bir biyopsi işlemidir.
Clinical Application of Liver Biopsy Performed with Ultrasound Gıided Percutaneous Route: Kocaeli University Interventional Radiology Clinical Experience
Objective: Ultrasound-guided (US) percutaneous liver biopsy still represents the gold standard for the assessment of chronic liver disease and liver masses. Our study aims to evaluate the indications of liver biopsies performed in the last five years with ultrasound, characteristics of the needle used, other diseases of the patient, and demographic data in Kocaeli University, Faculty of Medicine, Clinic of Interventional Radiology. Methods: All patients undergoing percutaneous liver biopsy at our hospitalbetween January 2015 and May 2020 were evaluated for indications, complications and impact of histology. Results: Percutaneous liver biopsy was performed in 264 patients. Hepatitis B (HBV) (29.2%) is the most common indication among liver biopsy indications, and other indications are malignant masses (22%), non-alcoholic fatty liver disease (NAFLD) (11.7%), drug-induced liver disease (DILD) (11.4%), Alcohol (9.5%), hepatitis C (HCV) (9.5%), autoimmune hepatitis (6.8%). Conclusion: US-guided percutaneous liver biopsy is a quick, safe, effective, and minimally invasive biopsy procedure in- and outpatients.
