Multidisipliner Yoğun Bakım Ünitesinde Obstetrik Olgular: Retrospektıf Analiz

Amaç: Bu çalışmanın amacı, yoğun bakım ünitesine alınan obstetrik olguları retrospektif olarak değerlendirmek, yoğun bakım ünitesine kabul sıklığını, nedenlerini ve klinik sonuçlarını araştırmaktır.Gereç ve Yöntem: Gaziosmanpaşa Üniversitesi Araştırma Hastanesi yoğun bakım ünitesine 2007-2013 yılları arasında gebe veya postpartum dönemde yatırılan 15 obstetrik olgunun retrospektif demografik verileri, kabul nedenleri, yoğun bakımda yapılan girişimleri ve sonuçları değerlendirmeye alınmıştır.Bulgular: Dış merkezden yoğun bakım ünitemize sevk edilerek gelen hasta sayısı 10 (%66.6) idi. Olguların ortalama yaşı 28.80 ± 5.74’ dür. Yatış süreleri 3,20 ± 2,51 gündür. Yoğun bakım ünitesine kabulün en sık nedeni postpartum kanama idi. Olgulardan biri mortalite ile sonuçlandı. Mortalite oranı %6.7 olarak saptandı.Sonuç: Sonuç olarak obstetrik hastaların yoğun bakıma ihtiyacı sıklıkla obstetrik kanama ve kontrol edilemeyen hipertansiyon nedeniyle olmaktadır. Geliştirilen düzenli tedavi ve gebelik öncesi bakım hizmetleri ile maternal mortalite ve morbidite belirgin oranda azalacaktır.

Aim: The aim of this study is to retrospectively evaluate the obstetric cases who referred to intensive care unit, and define the frequency, cause and clinic outcomes of the patients. Material and Method: Demographic data, causes of reference, interventions in the intensive care and the outcomes of 15 obstetric cases in the pregnancy and postpartum period, whose referred to Gaziosmanpasa University Hospital Intensive Care Unit between 2007 and 2013 were included and retrospectively evaluated. Results: The frequency of patients who referred from another center to our intensive care unit was 10 (%66.6). The mean age of the patients was 28.80 ± 5.74. The mean hospital stay time was 3.20 ± 2.51. The most cause to refer into intensive care unit was postpartum hemorrhage. One of the cases was resulted in death. The mortality ratio was found as %6.7. Conclusion: In conclusion, the frequent cause of intensive care requirement of the obstetric cases were obstetric bleeding and uncontrolled hypertension. The maternal morbidity and mortality will be substantially decreased with advanced treatment modalities and maternal care before pregnancy.
