TÜBERKÜLOZ PERİTONİTİN EŞLİK ETTİĞİ BİR TÜBERKÜLOZ LENFADENIT VE
Ateş, kilo kaybı, sarılık, boyunda şişlik yakınma-sıyla başvuran 32 yaşındaki erkek olgunun akciğer radyogramında; bilateral, yaygın milier görünüm, boyun ve batın bilgisayarlı tomografisinde multipl lenfadenopati (LAP) ile batında asit saptandı. Servikal lenf bezi biyopsisi "kazeifiye granülomatöz inflamasyon olarak rapor edildi ve biyopsinin direkt yayma bakısında asido-rezistan basil pozitif bulundu. Parasentez yöntemiyle elde edilen periton sıvısının asido-rezistan basil kültürlerinde "Mycobacterium tuberculosis" üredi.Periton, boyun, mediasten ve batında yaygın nekroze LAP ve akciğerde milier dağılımla karakterize, yaygın tutulumlu, ağır form tüberkülozun günümüzde nadir görülmesi ve hastalığın sıra dışı görünümlerinin ayırıcı tanıda güçlükler oluşturabilmesi nedeniyle olgumuzu sunmayı uygun gördük.
MİLİER TÜBERKÜLOZ OLGUSU A CASE OF TUBERCULOSIS LYMPHADENITIS AND MILLIARY TUBERCULOSIS WITH TUBERCULOSIS PERITONITIS
A 32 year old male patient, suffering from fever, loosing weight, ichterius and swollen neck, has applied to our clinic. Chest X-RAY showed bilateral disseminated milliary patern, neck CT showed multiple LAP and abdominal CT showed multiple LAP and ascite. Cervical LAP biopsy was reported as caseous granulomatous inflamation and biopsy acid-fast staining was found positive. Culture of periton fluid for mycubacterium tuberculosis was positive. We present this case, because disseminated serious tuberculosis and disseminated LAP in peritoneum, neck, mediastinum, abdomen and milliary patern in lung are rarely observed.
