KRONİK OBSTRÜKTİF AKCİĞER HASTALIĞI OLAN HASTALARDA ST. GEORGE SOLUNUM ANKETİ–KOAH SPESİFİK FORMU (ST GEORGE RESPİRATORY QUESTİONNAİRE-COPD; SGRQ-C) İLE BODE İNDEKSİ ARASINDAKİ İLİŞKİ
Amaç: SGRQ- C (SGRQ-COPD), SGRQ'in içinden ölçümde zayıf olan 10 sorunun çıkarılmasıyla oluşturulan ve kronik obstrüktif akciğer hastalığı (KOAH) için spesifik olarak geliştirilen bir ankettir. Bu araştırmada SGRQ-C'nin Türkçe versiyonunun KOAH hastalarındaki geçerliliği ile birlikte BODE indeksi arasındaki ilişkiyi incelemeyi amaçladık. Yöntem ve Gereç: Hastanemiz polikliniğinde Ocak-Kasım 2012 tarihleri arasında KOAH tanısı alan ve GOLD kriterlerine göre evrelenen 80 hasta çalışmaya alındı. Hastalara SGRQ-C, KOAH Değerlendirme Testi (KDT), Vücut Kitle İndeksi ölçümü (VKİ), Solunum Fonsiyon Testi, 6 dakikalık yürüme testi (6DYT), Modifiye Medical Research Council (mMRC) Dispne skalası yapıldı ve BODE indeksi hesaplandı. Sonuçlar SPSS 16 programı ile analiz edildi. Bulgular: 74 hasta çalışmayı tamamlayabildi. Ortalama yaş 56,8±8,7, FEV1 % 45,3±16,7, VKİ 25,7±6,8 kg/m2, mMRC 2,8±0.9, 6DYT 320,2± 136,4 metre, BODE indeksi 4,9±2,6 idi FEV1 ile SGRQ-C Aktivite, Etki ve Toplam skor arasında orta derecede ve anlamlı (r=0,51-0,54), FEV1 ile SGRQ-C semptom skoru arasında düşük dereceli ve anlamlı korelasyon (r=0,45) saptandı. FEV1 ile KDT arasında düşük dereceli ve anlamlı (r=-0,43), SGRQ-C subskorları ile KDT skorları arasında iyi derecede ve anlamlı (r=0,77-0,84), BODE indeksi ve mMRC ile SGRQ-C Toplam skoru arasında orta derecede anlamlı korelasyon (r=0,55 ve r=0,65) izlendi. GOLD'a göre yapılan sınıflandırmada, gruplar arasında mMRC, BODE indeksi, KDT ve SGRQ-C Semptom, Aktivite, Etki ve Toplam skorlar bakımından istatistiksel olarak anlamlı derecede fark bulundu. Sonuç: SGRQ'e göre daha kısa olan SGRQ-C, KOAH hastalarında geçerli bulunmuştur. BODE indeksi, SGRQ-C toplam skoruyla anlamlı ve orta derecede korele bulunmuştur. SGRQ-C'nin survi ile ilişkisini değerlendirmek için SGRQ-C, BODE indeksi ile yapılacak kohort araştırmalara ihtiyaç vardır.
THE RELATION BETWEEN SAINT GEORGE RESPIRATORY QUESTIONNAIRE-COPD (SGRQ-C) AND BODE INDEX IN COPD PATIENTS
Aim: SGRQ-C was developed specifically for COPD patients by removing ten items from SGRQ . BODE index was shown to predict survival in cohort studies.We aimed to validate the Turkish version of SGRQ-C and determine the relation between SGRQ-C and BODE index in patients with COPD. Material and Methods: 80 patients diagnosed as COPD and staged according to GOLD criteria between January-November 2012 were enrolled to the study. SGRQ-C, COPD Assessment Test (CAT), Body Mass Index (BMI), Respiratory Function Test, 6 Minute Walking Test (6MWT), modified Medical Research Council (mMRC) dyspnea scale were done and BODE index was calculated. Results: Study was completed with 74 patients. The mean age, FEV1, BMI, mMRC, 6MWT and BODE index were 56,8±8,7, %45,3±16,7, 25,7±6,8 kg/m2, 2,8±0,9, 320,2±16,4 meters and 4,9±2,6 in order. Mean CAT score, SGRQ-C Symptom score, Activity score, Impact score and Total score were 18,2±6, 55,3±24,8, 59,9±26,2, 38,1±28,8 and47,7±25,5 in order. FEV1 was moderately correlated with SGRQ-C Activity, Impact and Total scores, while weakly correlated with Symptom scores. FEV1 was significantly but weakly correlated with CAT scores. SGRQ-C subscores were significantly and well correlated with CAT scores. SGRQ-C Total score was moderately correlated with BODE index. There were statistically significant differences in all the measurements among different GOLD stages. Conclusion: The Turkish version of SGRQ-C which is shorter and easier to perform than SGRQ is valid in COPD patients. BODE index is moderately correlated with SGRQ-C scores. Longitudinal studies with SGRQ-C and BODE index are needed to determine the predictive value of SGRQ-C for survival
