İLGİNÇ RADYOLOJİK OLGU: YABANCI CİSİM

Yabancı cisim aspirasyonları özellikle çocukluk çağındaki acillerin ve kaza ile ölümlerin başında yer alır. Bulguları diğer solunum sistemi hastalık bulguları ile aynı olabilir. Ciddi komplikasyonlara sebep olarak cerrahi gerektirebilir. Anamnez büyük önem taşır. On bir yaşındaki erkek hasta kliniğe bir haftadır devam eden nefes darlığı ile başvurdu. Yabancı cisim aspirasyon öyküsü vermeyen hastada hafif siyanoz mevcut idi. Akciğer grafisinde sol hemitoraksta totale yakın şekilde homojen dansite artışı, bilgisayarlı tomografide sol akciğerde bronkogramlar, hava kistleri, atelektatik alanlar, total pnömonik konsolidasyon gözlendi. Tanı amaçlı yapılan rijid bronkoskopide sol ana bronş girişinde yabancı cisim (zeytin çekirdeği) tespit edilerek çıkarıldı. İşlem sonrası hastanın kliniğinde ve radyolojik görüntülerinde akut düzelme gözlendi. Solunum sistemi ile ilgili semptomlar ve radyolojik tetkikler yabancı cisim aspirasyonu ve diğer solunum yolu hastalıkları ile kolaylıkla karışabilmektedir. Şüphelenildiğinde bronkoskop yardımı ile ayırıcı tanısı hızla yapılmalı ve yabancı cisim çıkarılmalıdır.

AN INTERESTING RADIOLOGICAL CASE: FOREIGN BODY

Foreign body aspiration is a common emergency especially in childhood. It has a high mortality rate among accidentally deaths. Aspirations could have similar clinical findings with other pediatric pulmonary problems. This may necessiate surgery. History takes a great role in diagnosis. An 11 years old patient admitted to the clinic with symptom of dyspnea persisting for one week. He was mildly cyanotic on physical examination. There was no spesific history for diagnosis. Chest x-ray showed a totally homogenous increased dansity at the left hemithorax and computed tomography revealed air bronchograms and cysts at left lung, there was a total consolidation like pneumonia and atelectatic parts. Diagnostic broncoscopy revealed a foreign body (olive pip) which was obstructed the left main bronchus. It was removed by means of broncoscope. In some cases, clinical and radiological findings can not have the utility of differentiating foreign body aspiration from other respiratory system diseases. If there is any suspicion, the patient should be evaluated promptly and systematically and treated as soon as possible by using a broncoscope before medical therapies.
