GÖĞÜS DUVARINDA PLEVRAL EFÜZYON SONRASI GÖRÜLEN TÜBERKÜLOZ SOĞUK ABSESİ

irmibir yaşında erkek hasta sağ yan ağrısı yakınması ile hastanemize yatırıldı. Hastanın çekilen P/A akciğer grafisinde sağ kostofrenik sinüs küntlüğü saptandı. Toraks ultrasonografi (USG) ile plevral efüzyonun yeri işaretlenmesine rağmen plevral ponksiyon ile sıvı alınamadı. Bir ay sonra hastada sağ lateral göğüs duvarında şişlik gelişti. Toraks USG eşliğinde lezyona ponksiyon yapıldı. Yaklaşık 5 cc apse alındı. Mikrobiyolojik incelemede asidorezistan basil (ARB) saptandı. Daha sonra ARB kültürü pozitifdi. Altı aylık dörtlü antitüberküloz tedavi sonrası lezyon kayboldu. Tüberküloz soğuk absesi tanısı konan olgu, nadir görülmesi nedeniyle literatür eşliğinde tartışıldı.

TUBERCULOUS COLD ABSCESS OF CHEST WALL AFTER PLEURAL EFFUSION

A 21 years old male patient with symptom of right chest pain was admitted to the our hospital. It was revealed blunting of right costophrenic angle by chest x-ray screening. Although the place of the effusion marked by thorax ultrasonography (USG), there isn't pleural fluid with thoracentesis. One month later, swelling on right lateral chest wall was developed. Needle aspiration was performed on swelling with thorax USG guidance. Abscess was taken up to 5 cc. Microbiologic examination revealed acid-fast bacilli (AFB). Then, AFB culture was positive. The patient received antituberculous treatment with four drugs for six months and the lesion disappeared. We presented the case along with literature, as tuberculosis cold abscess of chest wall is rarely seen.
