ASBEST KULLANIMININ YAYGIN OLDUĞU BİR BÖLGEDE KADINLARDA MEZOTELYOMANIN KLİNİK VE DEMOGRAFİK ÖZELLİKLERİ
Malign Mezotelyoma (MM) etiyolojisinde genellikle çevresel ve mesleksel asbest maruziyeti rol alır. Ayrıca yine bir fibröz zeolit olan erionit de suçlanmaktadır. Ülkemizde çevresel asbest ve erionit teması nedeniyle MM yaygın bir kanser türüdür. Çalışmamızda 2005-2011 yılları arasında hastanemizde MM tanısı konan 77 kadın hasta retrospektif olarak değerlendirildi. MM hastalarının dosyaları ayrıntılı olarak incelendi. Çalışmaya alınan hastaların demografik verileri, yaş, şikayet, radyolojik bulgular, tanı yöntemleri, hastalık evreleri daha önceden hazırlanan standart formlara kaydedildi. MM hastalarının yaş ortalaması 57 ± 14.1 yıl idi. Elli yedi hastada (%74) olguda çevresel asbest maruziyeti mevcuttu ve ortalama temas süresi 30.6 yıldı. Hastalardan 21'i (%27.3) asbest kullanımının yaygın olduğu Ergani ilçesinden başvurmuştu. Toplam 63 hastada (%81.8) nefes darlığı, 44 hastada (%57.1) göğüs ağrısı ve 16 hastada (%20.8) kilo kaybı vardı. Ortalama semptom süresi 5.7 ± 4.6 aydı. Ayrıca 47 hastada (%61) histopatolojik tip olarak epitelyal tip olarak saptandı. Hastalarda ortalama sağ kalım 9.54 ± 6.9 ay olarak bulundu. Malign mezotelyoma çevresel asbest maruziyetinin fazla olduğu bölgelerde yaşayan kadınlarda uyumlu semptom ve bulguları olanlarda akla getirilmelidir. Bu kişilerde özellikle plevral tutuluma dikkat edilmelidir. Ülkemizde bu hastalığın önlenmesi için çevresel asbest kullanımının tamamen terk edilmesi için çalışmalar yapmak faydalı olabilecektir.
CLINICAL AND DEMOGRAPHIC CHARACTERISTICS OF WOMEN MESOTHELIOMA IN COMMON USE OF ASBESTOS IN THE REGION
Malignant mesothelioma (MM) in the etiology of environmental and occupational exposure to asbestos often plays a role. In addition, it is a fibrous zeolite erionite the accused. MM due to environmental asbestos and erionite is a common theme in our country is a type of cancer. In our study, 77 female patients diagnosed with MM at our hospital between 2005-2011 were evaluated retrospectively. MM patients were examined in detail in the files. The demographic data of the patients, age, complaints, radiological findings, diagnostic methods, the standard forms prepared in advance stages of disease were recorded. The mean age of MM patients was 57 ± 14.1 years. Fifty-seven patients (74%) patients had environmental asbestos exposure and the average exposure time is 30.6 years. 21 patients (27.3%), the use of asbestos was commonly applied to the Ergani district. Total of 63 patients (81.8%) dyspnea, 44 patients (57.1%) chest pain and 16 patients (20.8%) had weight loss. The average duration of symptoms was 5.7 ± 4.6 months. In addition, 47 patients (61%) histological type was the epithelial type. The mean survival was found to be 9.54 ± 6.9 months. Malignant mesothelioma should be considered in patients who women living in areas with more than environmental exposure to asbestos, with compatible symptoms and signs. These people should be especially careful of pleural involvement. In our country, for the prevention of this disease to be completely abandoned the use of environmental asbestos studies will be useful to do.
