KRONİK HEPATİTLERDE DEMİR BİRİKİMİNİN HİSTOLOJİK AKTİVİTE İNDEKSİ KNODELL İLE KARŞILAŞTIRILMASI

Giriş ve Amaç: Kronik hepatitlerin seyrinde ve izleminde, karaciğerde boyanabilir demir depolanmasının bir gösterge olup olmayacağını araştırmak istedik. Başlangıç noktamız, kronik karaciğer hastalıklarında hepatotrop virüsler tarafından başlatılan hepatosellüler hasarın parankimal ve ekstraparankimal demir depolanmasına yol açabileceği ve demir depolanması ile yangısal yanıt şiddeti, fibrozis ve siroz arasında ilişki olabileceği idi. Gereç ve Yöntem: Hastanemizde, klinik ve serolojik olarak kronik hepatit ve/veya siroz tanısı alan 83 olgu 66 B , 17 C hepatit çalışma kapsamına alındı. Bu olgularda boyanabilir demir birikimini, hücre ve lobül düzeyinde dağılımını ve bunların hastalığın şiddeti grade , evresi stage ve tipi B-C ile ilişkisini araştırdık. Bulgular: Çalışmamızda B ve C hepatitlerde, demir birikimi bulunan ve bulunmayan gruplar arasında, demir birikiminin lokalizasyonu p=0.01 , niteliği p=0.14 ve yoğunluğu p=0.01 açısından istatistiksel olarak anlamlı bir fark olmadığı görüldü. B hepatitler için; demir birikiminin yoğunluğu ile, toplam histolojik aktivite indeksi Knodell skoru p=0.52 , hastalığın şiddeti grade p=0.41 ve evresi stage p=0.92 arasında istatistiksel olarak anlamlı bir ilişki izlenmezken, C hepatitler için; demir birikiminin yoğunluğu ile histolojik aktivite indeksi p

THE COMPARISON OF IRON DEPOSITION WITH HISTOLOGICAL ACTIVITY INDEX KNODELL IN CHRONIC HEPATITIS

Background and aim: We planned a retrospective study to evaluate if stainable iron deposition could be a parameter in prognosis and follow-up of chronic viral hepatitis. Our starting point was the hepatocellular damage initiated by hepatotrophic viruses may lead to paranchymal and extraparanchymal iron deposition and there could be a relation between iron deposition and necroinflammatory grade, fibrosis and schirrosis in chronic liver disease. Material and method: 83 cases 66 B and 17 C hepatitis proved to be chronic hepatitis and/or schirrosis with clinical and serological data in our hospital were included in the study. We evaluated the stainable iron distrubition among cells and lobules and its relationship with disease grade, stage and virus type B-C . Results: Statistically, we didn’t found any correlation between localization, quality and intensity of iron deposition in B and C hepatitis cases for stainable/unstainable groups p=0.01, p=0.14 ve p=0.01, respectively Although there is no correlation between intensity of iron deposition and total HAI, necroinflammatory grade and stage of disease in B hepatitis p=0.52, p=0.41, p=0.92, respectively , a statistically significant correlation between intensity of iron deposition and HAI and stage of disease was found in C hepatitis p
