PERİASETABULER EWİNG SARKOMU OLGU SUNUMU: CERRAHİ TEKNİK VE LUMIC ASETABULAR İMPLANT İLE REKONSTRÜKSİYON

Pelvik tümörlerin rezeksiyonu sonrası meydana gelen periasetabuler defektlerin rekonstrüksiyonu ortopedik onkolojikcerrahide oldukça zo rlu b ir duru mdur. Geleneksel rekonstrüksiyon yöntemleri mekanik ve fonksiyonel açıdan yetersizkalabilmektedir. LUMIC® (Implantcast, Buxtehude, Germany) asetabuler implant, pelvik rezeksiyon sonrasırekonstrüksiyonda iyi sonuçların elde edildiğ i bir imp lant olarak kullanılmaktadır. Bu olgu sunumunda, 28 yaşındaperiasetabuler Ewing sarkomu sebebiyle tip-II pelvik rezeksiyon ve Lumic asetabular implant ile rekonstrüksiyonuygulanmış bir kadın hastanın klinik ve fonksiyonel sonuçları sunulmaya çalışıld ı. Bu olgunun diğer bir özelliği ise tip2 asetabuler rezeksiyonun trokanterik osteotomi yapılmadan gerçekleştirilmiş olmasıydı. Bu sebeple erken postopdönemde ve takiplerinde (2. yıl) aktif kalça abduksiyonu korunabildi ve Musculoskeletal Tu mor Society fonksiyoneldeğerlendirme skalasına göre iyi kalça fonksiyonu elde edileb ild i. Sonuç olarak LUMIC® acetabuler implant ilememnun edici klinik ve radyolojik sonuçlar elde etmek mümkündür. Tip 2 asetabuler rezeksiyon vakadan vakayadeğişebilmekle birlikte trokanterik osteotomi yapmadan uygulanabilir o lduğu gösterilmiştir

PERIACETABULAR EWING’S SARCOMA: A CASE REPORT OF SURGICAL TECHNIQUE AND RECONSTRUCTION WITH LUMIC ACETABULAR IMPLANT

Reconstruction of periacetabular defects following pelvic tumor resection is a demanding procedure in orthopaedic surgical oncology. Conventional reconstruction procedures may have dissatisfying mechanical and functional results. More recently developed, Lumic acetabular implant (Implantcast, Buxtehude, Germany) has been used for acetabular reconstruction after type 2 pelvic resection. We aimed to present a 28-year-old female patient who underwent a reconstruction surgery with a Lumic acetabular implant following a type-II pelvic resection due to periacetabular Ewing’s sarcoma. Another important feature of the patient is that the surgery was performed without trochanteric osteotomy. Therefore in the early postoperative period and the second postoperative year, the patient was capable for active hip abduction with a good fonctional score according to Musculoskeletal Tumor Society Rating Scale. In conclusion, satisfactory functional and radiological results can be obtained with Lumic® acetabular implant. It is also shown that type 2 resection can be achieved without trochanteric osteotomy.
