ASTIM HASTALARININ TAKİBİNDE ALERJİ DEĞERLENDİRMESİNİN ÖNEMİ

Bu çalışmada göğüs hastalıkları polikliniğine başvuran astım hastalarının alerji açısından değerlendirmesi amaçlandı. Astım tanısı ile göğüs hastalıkları polikliniğine başvuran 18 yaş üzeri 240 astım hastası retrospektif olarak değerlendirildi. Hastaların yaş, cinsiyet, eğitim durumu, çalışma durumu, ek hastalık, kronik hastalık, Total IgE sonucu, eozinofil yüzdesi, deri prik testi, astım şiddeti ve astım kontrol testi (AKT) sonuçları kaydedildi. Astım şiddeti ve AKT ile hastaların eğitim durumu, çalışma durumu, ek hastalık varlığı, kronik hastalık varlığı, deri prik testi ve Total IgE sonucu arasında istatistiksel fark saptanmadı. AKT ile eozinofil yüzdesi arasında ilişki saptanmazken; astım şiddeti ile eozinofil yüzdesi ilişkiliydi (p=0.039). Deri prik testi sonucuna göre alerjen olarak en sık ev tozu, polen ve hayvan tüyü saptandı. Astım hastalarında alerjinin erken dönemde saptanması hastaların takip ve tedavisi açısından çok önemlidir. Eozinofil yüzdesi astımda gelecek riskleri belirleme yönünden yarar sağlarken, Total IgE sonucu ise anti IgE (Omalizumab) tedavisi konusunda yol gösteren bir tetkiktir. Tedaviye dirençli ağır astım hastalarında periferik kanda eozinofil ve Total IgE mutlaka bakılmalı, uygun hastalar ileri merkezlere yönlendirilmelidir.

The Importance of Evaluation of Allergy in the Follow-Up of Asthma Patients

In this study, it was aimed to evaluate asthma patients who applied to the chest diseases outpatient clinic in terms of allergies. 240 asthma patients over the age of 18 who applied to the chest diseases outpatient clinic with the diagnosis of asthma were evaluated retrospectively. No statistical difference was obtained between the education status, employment status, presence of additional disease, presence of chronic disease, skin prick test and Total IgE results in terms of asthma severity and AKT. While no correlation was obtained between AKT and eosinophil percentage; asthma severity was associated with the eosinophil percentage (p=0.039). According to the results of the skin prick test, the most common allergens were detected to be house dust, pollen and animal feather. Early detection of allergy in asthma patients is very important for the follow-up and treatment of patients. While the eosinophil percentage provides benefits in determining future risks in asthma, the Total IgE result is a test that guides to anti-IgE (Omalizumab) therapy. In the patients with severe asthma which is resistant to treatment, peripheral blood eosinophils and Total IgE should be checked, and appropriate patients should be referred to advanced centers.
