Seröz vücut sıvılarının akım sitometri ile incelenmesi: Tek merkez deneyimi

Giriş: Güncel pratikte seröz vücut sıvıları biyokimyasal, mikrobiyolojik ve sitomorfolojik incelemeler ile değerlendirilmektedir. Seröz vücut sıvıları infeksiyöz ve inflamatuar hastalıkların yanı sıra hematopoetik ve solid neoplastik hastalıklar sonucu da oluşabilmektedir. Hematopoietik hücrelerin immünfenotipik olarak hızlı değerlendirilmesinde kullanılan akım sitometri yöntemi, özellikle nedeni bilinmeyen seröz vücut sıvılarının irdelenmesinde alternatif bir tanı yöntemidir. Bu çalışmada 4 yıllık süreçte, seröz vücut sıvılarının irdelenmesinde akım sitometri deneyimimizi gözden geçirdik. Materyal ve Metod: Mayıs 2010 - Şubat 2014 tarihleri arasında hastanemiz özel hematoloji bölümü laboratuvarında altı-renkli akım sitometri cihazı ile analizi yapılmış olan 77 hastanın (40 kadın, 37 erkek, ortanca yaş: 56 (20-84)), 80 seröz vücut sıvı örneği (45 plevra, 35 periton) retrospektif olarak incelendi. Bulgular: Kırkbeş plevra sıvı örneğinden 5'inde, 35 periton sıvı örneğinden ise 4'ünde hematolojik neoplazi için tanısal immünfenotipik bulgular gözlendi. Bu olgularda tanı ve alt tip tayini doku biyopsisi ve/veya sitomorfolojik analiz ile doğrulandı. Tartışma: Özellikle agresif seyreden ve zamanın hayati değer taşıdığı hematolojik kanserlerde patolojik değerlendirme nispeten uzun sürebilmekteyken akım sitometri ile seröz vücut sıvılarının immünfenotipik analizi daha erken tanıya ulaşmayı sağlayabilir kanısındayız. 

Investigation of serous body fluids with flow cytometry: single center experience

Introduction: In current clinical practice serous effusions are evaluated by cytomorphological, biochemical and microbiological studies. As well as infections and inflammatory diseases, serous body fluids can also be observed in hematopoietic and solid neoplastic diseases. Flow cytometry (FC) which can be used for evaluation of hematopoietic cells rapidly is an alternative diagnostic tool to examine unexplained serous body effusions. In this study, we reviewed our experience of flow cytometry for evaluating the serous body fluids during 4 years period. Materials and Methods: From May 2010 to February 2014, a total of 80 consecutive serous effusion specimens (45 pleural, 35 peritoneal) of 77 cases (40 female, 37 male, median age: 56 (20-84)), that were analysed by the six-color FC in the division of hematology laboratory at Akdeniz University Hospital, were investigated retrospectively. Results: Five of 45 pleural fluid samples and 4 of 35 peritoneal fluid samples were observed diagnostic immunphenotypic findings for hematologic neoplasia. Diagnosis was verified by tissue biopsy and/or cytomorphological analysis in these cases. Discussion: Cytopathologic evaluation may be delay especially in the diagnosis of aggressive lymphomas which requires urgent evaluation and treatment. In this case, immunophenotypic analysis of serous body fluids by FC can provide to access early diagnosis and also accurate clinical staging.
