EMBRİYO TRANSFER ÖNCESİ ENDOMETRİAL KO-KÜLTÜR UYGULAMALARI

Amaç: Embriyo implantasyon başarısını etkileyen faktörler arasında, embriyo kalitesi, endometrial reseptivite, ve embriyo transfer ET tekniği başlıca sayılabilecek etkenlerdir. İn-vitro fertilizasyon uygulamalarından sonuç alamayan infertil kadınlarda başarıyı olumlu yönde etkilemek için yeni yöntemlere ihtiyaç duyulmuştur. Günümüzde ko-kültür uygulamaları, iyi kalite embriyo transferine rağmen ≥3 Tekrarlayan İmplantasyon Başarısızlığı olan kadınlar için olumlu sonuca götürebilen önemli bir seçenektir. Endometrial ko-kültür ortamları ‘yapay uterus’kavramında da olduğu gibi, embriyoların transfer dönemi öncesi doğala yakın bir ortamda gelişimlerine izin veren sistemlerdir. Bu amaca yönelik, IVF kültür medyumlarına ilave olarak endometrial dokudan hazırlanan ko-kültür sistemleri tercih edilebilmektedir. Ana bulgular: Endometrium ile embriyo arasındaki etkileşimin en yüksek düzeyde olduğu dönem, embriyonun implantasyon dönemi olup, embriyo transferinin zamanlanması açısından da kritik bir süreçtir. Embriyo endometrium arası iletişimin 4, 5 ya da 6.gün olması nedeniyle, bu dönemde Embriyo Transferi ET yapılması daha avantajlı gibi görünmektedir. Bu uzun süreyi sağlamak için, in vivo şartları mümkün olduğunca taklit edebilen kültür sistemlerine ihtiyaç duyulmaktadır. Sonuç: Ko-kültür sistemlerinin embriyo gelişimine pozitif etkisi invivo şartları başarı ile taklit etmesinden kaynaklanmaktadır. Bu derlemede, yardımcı üreme tekniklerinde YÜT ko-kültür uygulamalarının gerekliliği ve sonuçları ele alınmıştır

Endometrial coculture applications before embryo transfer

Objective: It can be considered that the factors which affect the success of embryo implantation, embryo quality, embryo transfer ET technique and endometrial receptivity are the main ones. New methods were applied with the practices of invitro fertilization for the women who failed for positive impact on the success. Today, coculture application is an important option that can take a positive outcome for women with≥ 3 recurrent implantation failure RIF despite the transfer of good quality embryos.Endometrial coculture ambiance, the artificial uterus as discourse andthis system allows development embryos that are prior to the transfer embryos in natural environment. For this purpose, in addition to IVF culture medium, endometrial tissue coculture systems can be preferred.Main findings: The period when the highest level of interaction between endometrium and embryo, the embryo implantation period is a critical process in terms of timing of embryo transferEmbryo implantation 4, 5 or 6th day due to the ET in this period seems to be more advantageous. This long-time is to ensure, culture systems that mimic in vivo conditions as possible are needed. Conclusion: There is a positive effect of coculture systems on the development of the embryo.The reason for this is that successfully mimicked in vivo conditions. In this review, the requirement and results of assisted reproductive techniques ART and coculture applications are discussed
