Bariatrik cerrahi adayı hastaların psikiyatrik açıdan değerlendirilmesi

Amaç: Obezite dünyanın her yerinde yaygın olarak rastlanan, mortalite ve morbiditeyi arttıran ciddi bir halk sağlığı sorunudur. Bariatrik cerrahi ise son on yıl içerisinde giderek popüler hale gelen ve daha çok tercih edilmeye başlanan bir obezite tedavi yöntemidir. Bu çalışmanın amacı bariatrik cerrahi adayı olan obezite hastalarının psikiyatrik görüşme yapılarak ve psikometrik testler kullanılarak değerlendirilmesidir. Gereç ve Yöntem: Bariatrik cerrahinin psikiyatrik uygunluğunun değerlendirilmesi amacıyla tarafımıza danışılan 48 obezite hastası çalışmaya dahil edildi. Araştırmaya dahil edilen hastalara sosyodemografik veri formu, Beck Depresyon Ölçeği (BDÖ), Beck Anksiyete Ölçeği (BAÖ), Rosenberg Benlik Saygısı Ölçeği (RBSÖ), Başa Çıkma Tutumları Değerlendirme Ölçeği (COPE), Hollanda Yeme Tutumu Anketi (DEBQ) ve Barratt Dürtüsellik Ölçeği-11 Kısa Formu (BIS-11) uygulandı. Bulgular: Çalışmaya dahil edilen hastaların 32’si (%66.7) kadın ve 16’sı (%33.3) erkeklerden oluşmaktadır. Bariatrik cerrahi adayı hastaların vücut kitle indeksi (VKİ) ortalaması 45.23±8.97 olarak bulundu, cinsiyetler arası VKİ ortalamalarında fark saptanmadı. BDÖ puanlarında cinsiyetler arasında istatistiksel olarak anlamlı fark saptanmadı, BAÖ puanları kadınlarda yüksek bulundu. COPE fonksiyonel olmayan başa çıkma alt grup puan ortalamalarının kadınlarda istatistiksel olarak anlamlı düzeyde yüksek olduğu saptandı. DEBQ, RBSÖ-benlik saygısı alt ölçek ve BIS-11 değerlendirmelerinde cinsiyetler arasında anlamlı fark saptanmadı. Hastalar obezite ve morbid obezite olmak üzere iki gruba ayrılıp BDÖ ve BAÖ puan ortalamaları karşılaştırıldığında obezite grubunda her iki ölçek puanlarının morbid obezite grubundan yüksek olduğu saptandı. Sonuç: Obezite ruhsal bileşenlerin olduğu multisistem bir bozukluktur. Obezitenin ruhsal bileşenlerini ve bunların cinsiyetler arası farklılıklarını dikkate almak hastanın gerek cerrahi gerekse cerrahi dışı yöntemlerle tedaviye uyumunu kolaylaştırmada ve yinelemelerin önlenmesinde önemli bir basamak olarak değerlendirilmelidir.

Objective: Obesity is a worldwide public health problem, which increases morbidity and mortality. Bariatric surgery has become increasingly popular in the last decade and is a preferred treatment for obesity. The aim of this study is to evaluate the obese patients who are candidates for bariatric surgery by psychiatric clinical interview and psychometric tests. Material and Methods: 48 obese patients who were consulted to evaluate the psychiatric suitability of bariatric surgery were included in the study. Sociodemographic data form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Rosenberg Self-Esteem Scale (RSES), Coping Strategies with Stress Inventory (COPE), Dutch Eating Behavior Questionnaire (DEBQ) and Short Form of Barratt Impulsiveness Scale (BIS-11) were applied to the patients included in the study. Results: Of the patients included in the study, 32 (66.7%) were female and 16 (33.3%) were male. The mean body mass index (BMI) of the patients who are bariatric surgery candidates was found to be 45.23 ± 8.97, there was no difference between the two groups. There was no statistically significant difference in BDI scores between genders, and BAI scores were higher in women. COPE nonfunctional coping subgroup scores were found to be significantly higher in women. There was no significant difference between the sexes in the evaluations of DEBQ, RSS-self-esteem subscale and BIS-11. When the patients were divided into two groups as obese and morbidly obese, and mean scores of BDI and BAI were compared, it was found that both of the scores in the obese group were higher than the morbid obese group. Conclusion: Obesity is a multisystem disorder with mental components. Considering the mental components of obesity and their differences between genders should be evaluated as an important step in facilitating the patient's compliance with treatment with both surgical and non-surgical methods and in preventing recurrences.
