Majör tükürük bezi kitlelerinde ince iğne aspirasyon biyopsisinin tanısal değeri
Amaç: Majör tükürük bezi kitlelerinin tanısında ince iğne aspirasyon biyopsisinin (iiAB) değerinin tartışılması.Gereç ve Yöntem: Haziran 2005-Aralık 2008 tarihleri arasında kliniğimizde majör tükrük bezi kitlesi nedeniyle öpere edilen ve tetkik sonuçlarına ulaşılabilen 54 hastanın preoperatif iiAB sonuçlan ile operasyon materyalinin histopatolojik değerlendirmesi ile elde edilen sonuçlar karşılaştırıldı. iiAB'nin duyarlılık, özgüllük, tanı doğruluğu parotis, submandi-buler bez ve tüm hastalar için araştırıldı.Bulgular: iiAB'nin 24'ii (%44,4) parotis, 30'u (%55,5) submandibular kitleden yapılmıştır. Bu materyallerden 49'u (%90,7) benign, 3'ii (%5,55) malign, 2'si (%3,7) yetersiz materyal tanısı almıştır. Histopatolojik değerlendirme sonucu malign olduğu tespit edilen olgularda iiAB ile iki olguda yalancı negatif sonuç elde edilirken, 1 olguda gerçek pozitif sonuç elde edilmiştir. Tüm olgularda duyarlılık, özgüllük ve doğruluk oranlan sırasıyla %33,3 %100, %92,6 olarak elde edilmiştir.Sonuç: Majör tükrük bezi kitelerinde iiAB sonucunun benign olması ma-lignite ihtimalini ortadan kaldırmaz. Bu açıdan hastanın değerlendirilmesinde klinik bulgular, ultrasonografı, bilgisayarlı tomografi gibi diğer tanı metodlarmın bulguları ve iiAB sonuçları birlikte kullanılmalıdır.
Purpose: To discuss the value of fine needle aspiration biopsy in the diagnosis of major salivary gland masses.Material and Method: Among the patients operated between 2005 and 2008, FNAB results of 54 patients were available and were compared to histopathologic diagnosis. Sensitivity, specificity and accuracy rate of FNAB were investigated for parotid, submandibular salivary glands and for the whole patient population.Results: FNAB was performed on 24 (44,4%) parotid and 30 (55,5%) submandibular salivary gland masses. Forty-nine of these masses (90,7%) were classified as benign, three (5,55%) were malignant, and two (3,7%) were reported as insufficient material. Among the cases determined to be malignant by histopathologic evaluation, there were two false negatives and one true positive result by FNAB. Sensitivity, specificity and correct diagnosis rates were 33,3%, 100%, 96,1%, respectively. Conclusion: A benign FNAB result does not necessarily exclude a malignancy in major salivary gland masses. So, the patients should be evaluated taking into account the clinical findings, FNAB results and other diagnostic methods like utrasonography, computed tomography.
