Hamstring Tendon Otogrefti ile Ön Çapraz Bağ Rekonstrüksiyonu

Amaç: Bu çalışmada semptomatik kronik ön çapraz bağ yırtığının otojen Hamstring tendon grefti kullanılarak yapılan artrosko¬pik rekonstruksiyonunun sonuçları incelenmiştir. Gereç ve Yöntem: 2006 − 2008 tarihleri arasında Fırat Üniversitesi Tıp Fakültesi Hastanesi Ortopedi ve Travmatoloji Anabilim Dalında artroskopik olarak ön çapraz bağ yırtığı rekonstrüksiyonu yapılan 52 hasta incelendi. Tüm hastalar ortalama 20,5 (14 – 27) ay sonunda fonksiyonel olarak Lysholm diz skorlaması, IKDC (International Knee Documentation Committee) skorlaması ile klinik olarak diz eklem hareket açıklığı, stabilite testleri, uyluk çevresi ölçümü ve kişisel memnuniyet ile değerlendirildi. Bulgular: Lysholm skorlamasında 46 hasta (% 88,5) iyi ve mükemmel sonuç, IKDC skorlamasına göre 47 hasta (% 90,4) A veya B, 5 hasta (% 9,6) ise C olarak değerlendirildi. Radyografik değerlendirmede, 9 hastanın diz ekleminde hafif, iki hastada orta derecede dejeneratif değişiklikler gözlendi. Sağlam tarafla karşılaştırıldığında, hastalarda fleksiyon ve ekstansiyon kaybı gözlenmedi. Sonuç: Ön çapraz bağ yırtıklarının, dört katlı otojen Hamstring tendon grefti kullanılarak yapılan artroskopik rekonstrüksiyonlarında hem fonksiyonel olarak düzelme hem de hastalarda yüksek memnuniyet oranları tespit edilmiştir.

Anterior Cruciate Ligament Reconstruction with Hamstring Tendon Allograft

Objective: We evaluated the results of arthroscopic reconstruction of anterior cruciate ligament ruptures using autogenous Hamstring tendon autograft. Materials and Methods: We evaluated 76 patients undergo arthroscopic reconstruction of ACL tears whom are treated surgically between years 2006 − 2008 in Fırat University Hospital Department of Orthopaedics Clinics. All patients were evaluated with Lysholm knee score, IKDC (International Knee Documentation Committee) score for functional evaluation, range of motion of the joint, stability tests, thigh atropy measurements and subjective satisfaction for clinical evaluation, at the end of the postoperative 20,5 (14 – 27) month. Results: The Lysholm scores were good and excellent for 46 patients (% 88,5) and the IKDC scores were grade A or B in 47 patients (% 90,4 ) and grade C in 7 patients (% 9,6). Radiographic examination showed mild (nine patients) or moderate (two patients) degenerative changes in the knee joint. Compared with the normal side, no flexion or extension losses occurred in the affected knees. Subjective satisfaction of the patients was noted as 7,5. Conclusion: We conclude that arthroscopic reconstructions of anterior cruciate ligament with use of a four strand Hamstring tendon autograft which revealed successful functional results and also provided high personal satisfaction, is a reliable technique in the surgical treatment of anterior cruciate ligament ruptures.
