Çocuk acil polikliniğine karın ağrısı şikayeti ile başvuran hastaların değerlendirilmesi
Bu çalışmada, karın ağrısı şikâyeti ile hastaneye başvuran hastaların karın ağrısı nedenlerini, eşlik eden semptomları, etken patojenlerin belirlenmesi ve gerekli tedavi yaklaşımlarının tartışılması planlandı. Çocuk acil polikliniğine karın ağrısı şikâyeti ile başvuran, yaşları 2 ile 16 yıl arasında olan 626 hasta retrospektif olarak incelendi. En sık karın ağrısı nedeni olarak, %40,8 oranında akut gastroenterit saptandı. Bunu, üriner sistem enfeksiyonu %15, kabızlık %9,3, üst solunum yolu enfeksiyonu % 3,6 ve parazitozlar %3,6 oranı ile izledi. Acil cerrahi tedavi gerektiren akut karın ağrısı nedenleri %6,2 oranında bulundu. Karın ağrısı yanında 512 hastada (%81,8) ek semptom bulunmaktaydı. Karın ağrısına en sık eşlik eden semptomlar, ishal %38, iştahsızlık %29, kusma % 24, bulantı %16, ateş %16 olarak bulundu. Hastaların, kliniğimize başvuru sırasında alınan anamnezlerinde, karın ağrısının %55 hastada 0-24 saat, %22 hastada 24-72 saat, %11 hastada 72 saat-1ay, %7 hastada 1-6 ay ve %5 hastada 6 aydan fazla sürdüğü saptandı. Karın ağrısı şikâyeti, çok geniş bir hastalık grubunun semptomu olarak ortaya çıkabilir. Karın ağrısı şikâyeti ile başvuran hastaların ayrıntılı anamnezlerinin alınması ve dikkatli bir şekilde fizik muayenelerinin yapılması, hastaların büyük bir kısmında tanıyı koymada ve tedaviyi belirlemede yeterli olmaktadır.
Evaluation of patients with acute abdominal pain admitted to the pediatric emergency department
In this study, we aimed to evaluate causes and associated symptoms of acute abdominal pain in children applied to the Pediatric Emergency Department, to determine the pathogenic factors and to discuss approaches to the treatment. We evaluated retrospectively 626 children between the ages of 2 and 16 years. The most common cause of acute abdominal pain was acute gastroenteritis (40.8%), following by urinary tract infection (15%), constipation (9.3%), upper respiratory tract infection (3.6%) and parazitosis (3.6%). Emergency surgical treatment required in 6.2% of patients. 81.8% of the patients had associated symptoms to abdominal pain. The most common associated symptoms were diarrhea (38%), decreased appetite (29%), vomiting (24%), nausea (16%) and fever (16%). Duration of abdominal pain according to the anamnesis taken during the application was 0-24 hours in 55%, 24-72 hours in 22%,7 2 hours-1 month in 11%, 1-6 months in 7% and >6 months in 6% of the patients. Abdominal pain may occur as a symptom of a large group of diseases. Detailed anamnesis and careful physical examination are sufficient in making diagnosis and determining treatment.
