AÇIK KAPANIŞ TEDAVİSİNDE MODİFİYE HIZLI MOLAR İNTRÜZYON AYGITININ KULLANIMI: VAKA RAPORU

Bu vaka sunumunda büyüme gelişimi devam eden açık kapanışlı olguda Modifiye Rapid Molar Intruder (MRMI) aygıtı ile posterior bölgede intruzyon etkisiyle elde edilen değişikliklerin incelenmesini amaçlanmaktadır. Bu uygulama Ankara Üniversitesi Diş Hekimliği Fakültesi Ortodonti Anabilim Dalına tedavi amacıyla başvuran 10,5 (yıl) yaşındaki kız bireyde gerçekleştirilmiştir. Maksilla ve mandibulada premolar ve molar dişleri içeren sağ ve sol olmak üzere birer çift (toplam 4 parça) akrilik splint hazırlanmıştır. Akrilik plaklar üst çenede transpalatal ark, alt çenede lingual ark vasıtasıyla birleştirilmiştir. RMI aygıtı posterior akrilik plaklar arasında dik yönde 550±50 gram kuvvet uygulayacak şekilde ayarlanmıştır. İlk kayıtlar uygulama başında (T1), ikinci kayıtlar ise uygulama sonunda (T2) toplanmıştır. Tedavi başında ve sonunda bireyden toplanan aynı kayıtlar (lateral sefalometrik, el-bilek, panoromik radyografiler ve ortodontik modeller) değerlendirilmiştir. Açık kapanış tedavisinde büyüme gelişimi devam eden bireyde uygulanan MRMI apareyinin, açık kapanışın eliminasyonunda etkili olduğu sonucuna varılmıştır.

Modified Rapid Molar Intruder Approach in the Treatment of Open Bite: Case Report

In this case report, it is aimed to investigate the skeletal and dental changes with the posterior dentoalveolar intrusion by the modified Rapid Molar Intruder (MRMI) device. Our patient is a10.5 year old female patient with an open bite who was admitted to Ankara University Faculty of Dentistry Department of Orthodontics for orthodontic treatment. Pair of double acrylicsplints were prepared including maxillary molar-premolar teeth and mandibular molarpremolar teeth (total of 4 pieces). The transpalatal arch in the maxilla was bilaterally applied through the lingual arch in the mandibula. The RMI device was adjusted to apply a force of 550 ± 50 grams perpendicularly between the posterior acrylic splint. The records were obtained at the beginning of the application (T1) and at the end of the application (T2). The same records (lateral cephalometric, hand-wrist, panoromic radiographs and orthodontic  model) obtained from the individual at the beginning and at the end of the treatment were evaluated. It can be concluded that the MRMI appliance applied to a growing patient for the open bite treatment is effective in the reduction of openbite.
