Yeni Koronavirüs Pnömonisi Önleme ve Kontrol Döneminde Ağız, Diş ve Çene Cerrahisi Acil Hastalarını Yönetme Deneyimi: Retrospektif Çalışma

GİRİŞ ve AMAÇ: Bu retrospektif çalışmanın amacı COVID-19 pandemi sürecinin başlangıç döneminde, fakültemize başvuran ve ağız, diş ve çene cerrahisi hekimleri tarafından muayenesi ve tedavisi gerçekleştirilen hastalarda, klinik şikayetlere göre acil tanımı ve uygulanan tedavi yöntemlerini değerlendirilmek ve yorumlamaktır. YÖNTEM ve GEREÇLER: Çalışma, pandemi ilan edilen 2020 Mart ile Nisan aylarında, Ege Üniversitesi Diş Hekimliği Fakültesi Triaj Kliniği’ne acil dental tedavileri için başvuran ve Ağız, Diş ve Çene Cerrahisi Anabilim Dalı hekimleri tarafından muayene ve tedavileri gerçekleştirilen hastalar üzerinde retrospektif olarak yürütülmüştür. Diş Hekimliğinde Acil Durum sınıflamasında bulunan tanılara sahip hastaların demografik bilgileri, başvuru nedenleri, oral bulguları ile uygulanan acil tedavi yöntemi ve alınan önlemleri içeren klinik takip verileri değerlendirilmiştir. İstatistiksel analiz, verilerin yüzde dağılımı (%) ve ortalama (±) standart sapma (SD) değerleri üzerinden yapılmıştır. BULGULAR: Toplam 95 hasta (41 kadın, 54 erkek; yaş ortalaması 38.51±16.80) çalışmaya dahil edilmiştir. Bu hastaların %45.3’ünün (N=43) acil durum sınıflandırılmasına göre tanısı lokalize ağrı ve şişmeye neden olan apse veya bakteriyel enfeksiyondur. Tedavi seçimi ise hastaların %60’ında (N=57) diş çekimi olmuştur. TARTIŞMA ve SONUÇ: Bu retrospektif çalışmada, operatif olmayan diş çekimi, aeresol oluşturma oranı daha düşük olan radikal bir tedavi seçeneği olarak pandemi döneminde triaj kliniğine başvuran ve acil tanımına uyan hastalarda en çok uygulanan tedavi seçeneği olmuştur. COVID-19 pandemisi esnasında elde edilen tecrübeler, her sağlık kuruluşu için ileride meydana gelebilecek küresel salgınlara örnek teşkil etmesi açısından uygun protokoller hazırlanması gerekliliğini gündeme getirmiştir. Bu nedenle, yerel otoriteler tarafından her uzmanlık dalına spesifik olarak hasta ve hekim güvenliğini içeren kılavuzlar hazırlanmasının, olası küresel salgın durumunda yol gösterici olacağı düşünülmektedir.

Management of Patients in Oral Maxillofacial Surgery during the Prevention and Control Period of the New Coronavirus Pneumonia: A Retrospective Study

INTRODUCTION: The aim of this retrospective study was to assess the dental emergencies and treatment methods according to clinical complaints in patients who referred to our faculty, and treated by oral and maxillofacial surgeons, during the initial period of COVID-19 pandemic. METHODS: This retrospective study was carried out the patients who referred to Triage Clinic of School of Dentistry, Ege University for dental emergency, and treated by the oral ad maxillofacial surgeons, between March and April 2020.All data were recorded as follows: demographic variables, recourse definition, oral findings, and clinical follow-up including the treatment and precaution methods. Data were classified as percentage (%), mean (±) and standard deviation (SD). RESULTS: A total of 95 patients (41 females, 54 males; mean age of 38.51 ± 16.80) were evaluated in the study. According to the Emergency Classification in Dentistry, 45.3% of the patients (N = 43) had oral abscess or bacterial infection related to localized pain and swelling. Treatment methods were defined as a tooth extraction in 60% of the patients (N = 57). DISCUSSION AND CONCLUSION: In this retrospective study, tooth extraction, as a radical treatment with nominal aerosol risk, was the most preffered treatment method in patients who referred to the Triage Clinic during COVID-19 pandemic. The earnings in COVID 19 pandemic brought to the agenda the necessity of preparing appropriate protocols for each healthcare center to set an example by future pandemics. Therefore, it is thought that the guidelines including the precautions related to patient and physician may provide a data for a possible epidemics in worldwide.
