Yenidoğan konvülsiyonları
izlenen 43 yenidoğan konvülsiyonlu (YDK) bebekte etiyolojik neden olarak sırasıyla perinatal asfiksiye bağlı olarak hipoksik iskemik ansefalopati (HİA) (%26); hipokalsemi(%23), hipoglisemi(%16), hipoglisemi + hipokalsemi (%7) saptanmıştır. Metabolik bozuklular daha çok prematüre, düşük doğum ağırlıklı ve asfiktik bebeklerde izlenmiş; YDK dan korunmak için perinatal asfiksiden kaçınma gereği vurgulanmıştır. HİA de konvülziyonlar erken (ilk üç günde), metabolik olaylarda sıklıkla erken, sepsisde ise sıklıkla geç olarak çıkmıştır. Metabolik nedenli konvülsiyonlar genelde organik nedenlilere göre daha kısa sürmüştür. En sık gözlenen konvülsiyon tipleri unifokal klonik (%27) ve multifokal klonik (%27) olarak izlenmiş, konvül¬siyon tipi ile etiyoloji arasında ilişki bulunmamıştır. Tedavide metabolik düzensizliğin giderilmesi ve fenobarbital uygulaması genellikle yeterli olmuş, bazı olgularda ikili, üçlü antikonvülsan gerekmiştir. İki olgu kaybedilmiş, düzenli kontrole gelen 14 olguda % 71 oranında sekel saptanmıştır. Sekeller mental motor gerilik, infantil serebrai felç (İSF), İSF+epilepsi şeklindedir.
Neonatal seizures
Forty-three neonates with neonatal seizures were followed up at the neonatal intensive care unit. The major etiological fac¬ tors were hypoxic ischaemic encephalopathy (HIE) due to perinatal asphyxia (26 %), hypocalcemia (23%), hypoglycemia (16%) and hypoglycemia+hypocalcemia (7%). Metabolic disturbances were mainly associated with prematurity, low birth weigth, and asphyxia; therefore the importance of prevention of birth asphyxia was stressed. Seizures were early-onset (within the first 72 hours) in HIE, mostly early-onset in metabolic disturbances and late-onset in sepsis. Seizures due to metabolic disturbances were mostly of shorter duration than those due to organic lesions. The incidence of unifocal, multifocal clonic seizures were 37% and 27% respectively with no association between the type of seizures and etiology. The elimination of metabolic disturbance and phenobarbital administration have been effective in the majority of patients with the need of dual or triple anticonvulsants in occasional cases. Two patients died, 71% of 14 patients who were followed up survived with sequelae. The observed sequelae were mental motor retardation, infantile cerebral palsy and infantile cerebral palsy + epilepsia.
