Yurdumuzda sağlıklı çocuklarda olduğu kadar hemofili hastalarında da en sık uygulanan operasyon sünnettir. Sünnet islam dininde oldukça önemli bir sosyal olgudur. Günümüzde hemofili tedavisinin en önemli prensibinin hastanın toplumdan ken-disini izole etmemesi ve aktif olarak toplumsal yaşamda yerini alması olduğu bilinciyle hemofili hastalarının sünnet olama-malarının getireceği psikososyal sorunlar ciddi boyutlara ulaşmaktadır. Buna karşın ileri yaşta bir hemofili hastasının faktör konsantreleri ile hastane koşullarında sünnet edilmesi, pahalı olması nedeniyle çoğu zaman mümkün olmamaktadır. Sün-netin deneyimsiz ve bilgisiz kişiler tarafından yapılması da ciddi komplikasyonlara neden olmaktadır. Bu merkezde edinilen deneyimler sonunda hemofili hastalarının sünnet edilmesi önemli bir sosyal destek olarak kabul edilmiş; güvenli, etkin ve ucuz olduğu kanıtlanmış bir protokol ortaya konulmuş ve diğer merkezlere önerilmiştir. Protokolün temel taşı fibrin yapıştırı-cısı "fibrin glue" nun lokal hemostaz desteğinde kullanılmasıdır.

Circumcision is the most common operation performed in either healthy boys or hemophiliacs in our country. Circumcision, a traditional procedure is an important rituel for Muslims. It is well known that social and cultural integration of hémophilie boys into society is one of the most important cornerstone of modern hemophilia therapy. Hence, hémophilie boys might fell compromised if they are not circumcised. Bleeding problems can be life-threating in case of circumcision performed by unexperienced person or insufficient replacement therapy. High cost of factor concentrates required for circumcision is the greatest obstacle to carrying out this procedure. Our center stated that circumcision is one of the important social problems to be solved for hémophilie boys, and we recommend safer, efficient and inexpensive protocol for circumcision. The basic point of this protocol is using fibrin glue for local hemostasis in circumcision.
