Polikistik over sendromlu hastalarda lipid parametrelerinin ve ailede diabetes mellitus sıklığının değerlendirilmesi

Amaç: Bu çalışmada polikistik over sendromu (PKOS) tanısı alan hastalarda lipid parametreleri ile hastaların aile ve akrabalarında Diabetes Mellitus (DM) sıklığı araştırılmıştır. Yöntemler: Kesitsel yapılan bu çalışmada Erzurum Bölge Eğitim ve Araştırma Hastanesinde PKOS tanısı almış 120 hasta ve 30 sağlıklı kontrol grubu çalışmaya dahil edilmiştir. Hastaların vücut kitle indeksleri, soy geçmişleri, ultrasonografik bulguları, kan hormon profilleri (Folikül stimulan hormon (FSH), Luteinizan hormon (LH), total ve serbest testesteron, prolaktin (PRL), Tiroid stimulan hormon (TSH)), lipid parametreleri (düşük dansiteli lipoprotein (LDL), yüksek dansiteli lipoprotein (HDL), trigliserid), plazma insülin seviyeleri, açlık kan glukoz değeri ve HOMA-IR (Homeostatic model assessment -insulin resistance) kaydedildi. Hastaların ailesinde ve akrabalarında DM olup olmadığı sorgulandı. Bulgular: Ailede DM hikayesi PKOSlu hastaların 29 (%24) unda, kontrol grubu hastalarının 2 (%6) sinde saptanmıştır. Uzak akrabalarda DM hikayesi PKOSlu grupta 45 (%35) hastada, kontrol grubunda ise 5 (%17) hastada saptandı. HOMA-IR değerleri, açlık kan şekeri ve insü- lin seviyeleri PKOS grubunda istatistiksel olarak anlamlı yüksek bulundu (p0,05). Sonuç: PKOS lu hastaların aile ve akrabalarında DM sıklığının kontrol grubuna göre istatistiksel olarak anlamlı şekilde daha fazla olduğu saptandı. Önceki çalışmaların aksine olarak bu çalışmada polikistik over sendromunda lipid parametrelerinde bozulma saptanmadı.

Evaluation of lipid parameters and family history of diabetes mellitus in polycystic ovary

Objective: In the present study we investigated lipid parameters of Polycystic Ovary Syndrome (PCOS) patients and Diabetes Mellitus (DM) history of their family and relatives. Methods: In this cross-sectional study, one hundred and twenty patients with PCOS and 30 healthy controls were involved at Erzurum Research and Training Hospital. History, Body mass index (BMI), ultrasonography findings, blood hormone profiles (Follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), thyroidstimulating hormone (TSH), total and free testosterone (Total-T and Free-T), prolactin (PRL)), and serum lipids (total cholesterol (Total-C), high-density cholesterol (HDLC), low-density cholesterol (LDL-C), and triglycerides (TG), fasting blood glucose and plasma insulin level were measured. HOMA-IR (Homeostatic model assessmentinsulin resistance) was calculated. Results: DM history in the family was found in 29 (24%) of PCOS patients and 2 (6%) of healthy controls. The difference between groups was statistically significant (p<0.05). DM history in the relatives was determined in 45 (35%) PCOS patients and 5 (17%) controls. The difference between groups was statistically significant (p<0.05). HOMA-IR, fasting glucose and insulin levels were significantly higher in PCOS patients (p<0.05). Lipid parameters were not significantly different between the two groups. Conclusion: Frequency of DM in the family and relatives of PCOS patients was significantly more common than controls. In contrast to previous studies, no deterioration was found in lipid parameters in patient with PCOS.
