Pilonidal sinüs tedavisinde rhomboid eksizyon ve limberg fleb onarımı: erken dönem sonuçları
Pilonidal sinüs sakrokoksigeal bölgenin yaygın görülen kronik bir hastalığıdır. Plonidal sinüs hastalığının tedavisi hastalığın prezentasyonu ile ilişkili olarak değişir. Buna göre birkaç cerrahi yöntem göstermiştir ki, en ideal yöntem halen yüksek nüks oranları nedeniyle bulunamamıştır. Biz pilonidal sinüs tedavisinde rhomboid eksizyon ve limberg fleb onarımının etkinliğini araştırdık. Gereç ve yöntem: Kronik ve subakut sakro koksigeal plonidal sinüs hastalığı nedeniyle, 32 (Erkek/Kadın 21/11) hastaya rhomboid eksizyon limberg fleb kapama tekniği uygulanmıştır. Ortalama hastalar 4 ila 9 ay takip edilmiştir. Hastalar; ameliyat süresi, hastanede yatış süresi, nüks oranı ve erken dönem komplikasyon oranları bakımından değerlendirilmiştir. Bulgular: Çalışmaya dahil edilen hastaların yaş ortalaması 28,8 ± 23 yıl, ortalama operasyon süresi 68 (52-77) dakika olarak kayıt edilmiştir. Ortalama hastanede yatış süresi 1 (1-3) gün, operasyon süresi ise 68 dakika (52- 77) hesaplanmıştır. Erken yara yeri komplikasyonu %6.2 (n=2) hastada kaydedilmiştir. Hastaların takip süresinde hiç nüks görülmemiştir. Sonuç: Sonuçlar sakrokoksigeal pilonidal sinüs tedavisinde romboid eksizyon ve Limberg flep kapamayı özellikle büyük vakalarda desteklemektedir. Düşük nüks oranı, kısa hastanede yatış süresi, düşük komplikasyon oranları ve işgücü kaybı nedeniyle, kötü kozmetik görünüme rağmen tercih edilebilir.
Rhomboid excision and limberg flap for managing pilonidal sinus: short-term results
Pilonidal sinus is a common chronic disease of the sacrococcygeal region. Treatment varies in relation to the presentation of the disease. Accordingly, several surgical methods show that the most ideal method is not still found because of high recurrence rates. We investigated the efficacy of rhomboid excision and Limberg flap in the treatment of pilonidal sinus. Materials and methods: Because of chronic and subacute sacrococcygeal pilonidal sinus disease rhomboid excision and Limberg flap closure technique was applied to 32 (Male / Female 21/11) patients. The median followup duration was 4 to 9 months. Operative time, hospital stay, recurrence rate and early complications were compared. Results: The mean age of the patients was 28.8 ± 23 years; mean operative time was 68 (52-77) minutes. The average hospital stay was 1 (1-3). Early wound complications were in 6.2% (n=2) patients. There was no recurrence during follow-up. Conclusion: The results show that rhomboid excision and Limberg flap closure technique can be used particularly for the treatment of large sacrococcygeal pilonidal sinus cases. It may be preferred for low relapse rate, short hospital stay, low complication rates and labor force loss, despite poor cosmetic results.
