Fahr hastalığı: Olgu sunumu
Fahr hastalığı; bazal ganglionlar, serebellar dentat nukleus ve sentrum semiovalede kalsifikasyon görülmesi ile karakterizedir. Klinik bulgular sıklıkla parkinsonizm, distoni, tremor, kore, ataksiye ek olarak demans ve duygu durum bozukluklarıdır. Biz de Fahr hastalığı saptadığımız olgumuzu klinik ve görüntüleme bulguları ile sunduk. Bir yıldır hafıza kaybı ve konuşmada bozulma yakınmaları 56 yaşındaki kadın hastanın nörolojik ve sistemik muayenesi normaldi. Kraniyal bilgisayarlı tomografi (BT) incelemesinde her iki bazal ganglionlarda, talamuslarda, periventriküler beyaz cevherde, sentrum semiovalede ve serebellumda çok sayıda kalsifiye alanlar izlendi. Sonuç olarak bazal ganglionlarda, serebellumda ve derin kortikal yapılarda kraniyal BT veya MR inceleme ile saptadığımız kalsifikasyonları herhangi bir nedene bağlayamadığımız durumlarda Fahr hastalığını ayırıcı tanıda düşünmeliyiz.
Fahr’s disease: A case report
Fahr’s disease is characterized with presence of calcifications in basal ganglions, dentate nucleus, and centrum semiovale. Common clinical findings of the disease are Parkinsonism, dystonia, chorea, ataxia, dementia, and mood disorders. We present a patient, in whom a diagnosis of Fahr disease established, with clinical and radiological findings. Neurological and physical exam of the 56 year-old female with complaints of memory loss and speech disorder for one year. Brain CT showed Fahr type calcification in the basal ganglion, thalamus, periventricular white matter, centrum semiovale, and cerebellum. Fahr’s disease must be present in differential diagnosis of patients with calcification in basal ganglion, thalamus, periventricular white matter, centrum semiovale, and cerebellum on CT or MRI that could not be explained otherwise.
