Hiperparatiroidizm nedeni ile ameliyat edilen hastalarımızın sonuçları

Hiperparatiroidizm nedeni ile 1998–2006 arasında Trakya Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalında ameliyat edilen 36 hastanın sonuçları retrospektif olarak değerlendirildi. Hastaların 26’sı (% 72) kadın 0’u (% 28) erkekti. Hastaların yaş ortalaması 46.6 (7–75) idi. Hastaların başvuru şikayetleri, kemik ve eklem ağrıları, osteoporoz, dizüri, dispepsi ve hipertansiyon idi. Preoperatif tanı amaçlı tüm hastalara boyun ultrasonografisi ve 99mTc-MİBİ ile paratiroit sintigrafisi, 7 (% 9) hastaya manyetik rezonans görüntülemesi, 4 (% ) hastaya da boyun bilgisayarlı tomografisi yapıldı. Tüm hastaların alkalen fosfataz, fosfor, kalsiyum, parathormon değerlerine bakıldı. Bilateral boyun eksplorasyonu yapılan hastalarımızdan biri, nüks hiperparatiroidi nedeniyle reopere edildi. Hiperparatiroidi tahmin edilenden çok daha sık rastlanılan ve kolay tanı konabilen bir hastalık olmasına rağmen klinisyenler tarafından aynı kolaylıkla gözden kaçabilmektedir. Hiperparatiroidi hastaları çok çeşitli şekillerde belirti vermeleri nedeniyle asıl tedavi edilecekleri klinikler dışında gereksiz olarak tetkik ve tedavi edilmeye çalışılırlar. Hiperparatiroidi hastalarının genel cerrahi, endokrinoloji, nükleer tıp ve radyoloji uzmanlarının oluşturduğu bir ekip tarafından multidisipliner anlayışla değerlendirilmeleri ve tedavi edilmeleri ideal yaklaşımdır.

The outcomes of our patients operated for hyperparathyroidism

In this study, 36 patients who underwent parathyroidectomy operation due to hyperparathyroidism between 998- 2006 in Trakya University Medical Faculty Department of General Surgery were retrospectively evaluated. There were 26 (72 %) female and 0 (28 %) male patients. The mean age was 46.6 (7-75 years). The complaints of the patients were bone and/or joint pains, osteoporosis, dysuria, dyspepsia, and hypertension. Preoperative diagnosis was made in all of the patients thorough the neck ultrasonographic evaluation and 99mTc-MİBİ parathyroid scintigraphy. Another 7 (9 %) patients had an additional magnetic resonance imaging scan, and 4 ( % 9) had an additional computed tomography scan of the neck. The levels of se-rum calcium, phosphorus, parathormone, alkaline phosphatase were investigated in all of the patients. All of the patients under-went bilateral neck exploration and one patient was reoperated due to recurrent disease. Although hyperparathyroidism is such an easy diagnosed and frequently seen illness, many clinicians dismiss this pathology. Patients with hyperparathyroidism tried to be investigated and treated in different clinics due to the large variety of symptoms. However patients with hyperparathyroidism should be treated by a multidisciplinary team consisting of general surgeon, endocrinologist, nuclear medicine specialist, and radiologist.
