Spontan akut subdural hematom
Spontan akut subdural hematom oluşumu nadirdir. Biz birisi komatöz, diğer ikisi bilinci açık olarak başvuran, kafa travması olmayan, hipertansif üç akut subdural hematom olgusu bildiriyoruz. Olgularda saptanan hematomlar Sylvian fissür çevresinde yerleşik idi. Öpere edilen iki olguda kortikal bir damardan kanama olduğu belirlendi; medikal destek tedavisi verilen diğer olguda ise angiografik olarak kanama ve vasküler patolojik lezyon saptanmadı. Arteriyel hipertansiyonun önemli bir predispozan faktör olduğu, yaşlı ve hipertansif kişilerde serebrovasküler hastalıkların ayırıcı tanısında akut spontan subdural hematomun hatırlanmasının gerekli olduğu vurgulandı.
Spontaneous acute subdural hematoma
Spontaneous acute subdural hematomas is very are. We presented two cases who were admitted with seizures, progressive neurologic deficits and consciousness disturbance, one becoming comatose. None had a history of head injury, but hypertensive, The subdural hematomas determined in cases were located in the adjacent Sylvian fissure. The bleeding of a cortical artery was determined in two operated cases. Any vascular pathological lesion or extravasation was not observed angiographycally in the case treated medically. It is emphasized that arterial hypertension is an important predisposing factor in occurrence of spontaneous subdural hematoma and a high index of suspicion for the rare entity should be kept mind in differential diagnosis of cerebrovascular accident.
