Diz Eklemi Osteoartritinde Eklem Kıkırdağının Değerlendirilmesi ve Diğer Osteoartritik Değişiklikler ile İlişkisi
Amaç: Osteoartriti (OA) olan hastalarda “Tüm Organ Manyetik Rezonans Görüntüleme skoru” kullanılarak OA’nın diz eklemine olan etkilerini vekıkırdak patolojisi ile eklemi oluşturan diğer komponentlerin arasındaki ilişkiyi değerlendirmektir.Gereç ve Yöntem: Diz ekleminde OA’ya bağlı ağrı yakınmaları olan 53 hasta 1.5 T manyetik rezonans (MR) cihazında görüntülenmiştir. Diz eklemianatomik ayırt ettirici noktalara göre 15 bölüme ayrılmıştır. Görüntüler değerlendirilirken 9 özelliğe skor verilmiştir. Bunlar; kartilaj sinyal vemorfolojisi, subartiküler kemik iliği anormalliği ve kistler, subartiküler kemikte düzleşme ve depresyon, marjinal osteofitler, ön ve yan bağlar, menisküspatolojileri, sinovyal kalınlaşma-eklem sıvısı ve eklem faresidir. Üç diz kompartmanından elde edilen değerlerin her biri ayrı ayrı hesaplanmıştır. Herbir kompartman için eklem yüzünü ilgilendiren özelliklerin kümülatif skorları ve her bir özelliğin dizin tüm kesimlerindeki toplam skorları eldeedilmiştir. Tüm diz için tüm özelliklerin skorları toplanarak toplam skor elde edilmiştir.Bulgular: Dizlerin %85’inde kıkırdak patolojisi saptanmıştır. Kıkırdak patolojilerinden en sık etkilenen bölge patellofemoral eklem, en az etkilenenbölge lateral femorotibial eklem olmuştur. Medial ve lateral femorotibial eklemde sıklık ve yoğunluk bakımından kıkırdak kaybından en fazlaetkilenen bölgeler santral kesimlerdir. Tüm bölgeler arasında kıkırdak patolojisinden en sık etkilenen alan patella medial faset, en az etkilenen alanise tibia lateral ön kesim olmuştur. Kıkırdak değerleri ile kemik iliği anormalliği, subartiküler kistler, osteofit formasyonları, subartiküler kemiktedüzleşme ve depresyon, menisküs skorları ve eklem sıvısı arasında korelasyon saptanmıştır.Sonuç: OA’lı hastalarda, MR’de kıkırdak defektlerine eşlik eden osseöz, sinovyal, meniskal, ligamentöz patolojiler görülebilir.
Evaluation of Joint Cartilage in Knee Joint Osteoarthritis and Relation with Other Osteoarthritic Changes
Objectives: To assess the relationship between cartilage lesions and the other components of the knee by using a scoring system called “Whole Organ Magnetic Resonance Imaging score” with magnetic resonance (MR) in the patients with osteoarthritis (OA). Materials and Methods: Fifty-three patients with symptomatic OA of the knee were recruited from the rheumatology clinic. MR studies of the knees were acquired with a 1.5 Tesla whole-body scanner using a commercial circumferential knee coil. Taking anatomical landmarks, the knee joints were evaluated in 15 different subregions and findings were scored for each region in fully extended position. Images were scored with respect to 9 independent articular features: cartilage signal and morphology, subarticular bone marrow abnormality, subarticular cysts, subarticular bone attrition, marginal osteophytes, medial and lateral meniscal integrity, anterior and posterior cruciate ligament integrity, medial and lateral collateral ligament integrity, synovitis and effusion and loose bodies. The final scores were tabulated as independent values for each feature in each of the three compartments of the knee, cumulative surface (cartilage, marrow abnormality, subarticular cysts, bone attrition, osteophytes) feature scores for each compartment, cumulative scores for each feature throughout the knee, and a total combined score for the entire knee. Results: Eighty-five percent of knees showed cartilage abnormalities. This was most frequent in the patellofemoral joint, but involvement of the lateral femorotibial joint was the least common. Among many of the individual features, particularly cartilage, bone cysts, bone attrition, osteophyte, effusion and meniscus were relatively strongly associated. Conclusion: Osseous, sinovial, meniscal and ligamentous pathologies may associate cartilage defects.
