Gebelerde epidural aralığın belirlenmesinde istemsiz dura ponksiyonu sıklığı: Retrospektif araştırma
Amaç: Gebelerde epidural aralığın belirlenmesinde istemsiz dura ponksiyonu (DP) ile girişim deneme sayısı ve anatomik yapı arasında bir fark olup olmadığının retrospektif olarak araştırılması amaçlandı. Yöntem: Elektif sezaryen ameliyatı için epidural anestezi uygulanan 972 gebede; epidural aralığı belirleme yöntemi, girişim deneme sayısı, ponksiyon yapılan aralık ve aralık değişimi, hastanın ponksiyon bölgesinin anatomik yapısı (belirgin, zor belirlenebilen, belirgin değil), kullanılan epidural iğne ve kateterin çapı, istenmeyen dura veya damar ponksiyonu insidansı, yan etkiler ile komplikasyonlar ve tedavide yaklaşımlar (konservatif veya epidural kan yaması: EKY) kaydedildi. Bulgular: Gebelerde (n=518 multipar ve n=454 primipar) daha çok 18 G epidural Tuohy iğnesi (% 95,9) ve 20 G çok delikli epidural kateter (% 73,4) tercih edildiği saptandı. Ponksiyon bölgesinde aralıkların anatomik yapısı gebelerin çoğunda belirgin (% 62,5) bulunurken, geri kalan gebelerde aralıkların ya zor (% 30,5) ya da hiç (% 7) belirlenemediği gözlendi. Epidural girişim için ponksiyon L3-L4 (n=807), L2-L3 (n=148) ve L4-L5’ ten (n=17) yapılmış, epidural aralığı belirlemede % 99,7 oranında direnç kaybı tekniği kullanılmış ve girişim gebelerin % 79,7’sinde ilk denemede başarıyla yapılmıştır. Cilt-epidural aralık mesafesinin 4,5 ± 0,7 cm olduğu bulunmuştur. istemsiz DP (n=12) ve kateter yerleştirilirken damar ponksiyonu (n=35) yapılan gebeler olduğu gözlendi. iki olgu tedaviyi kabul etmezken, 10 olguya konservatif tedavi uygulandığı ve tedaviye refrakter 2 olguda ise EKY yapıldığı belirlendi. istemsiz DP ile deneme sayısı ve anatomik yapı arasında; ayrıca girişim deneme sayısı ile aralıkların anatomik yapısı arasında istatistiksel olarak anlamlı fark saptanmadı (p > 0.05). Sonuç: Elektif sezaryenlerde epidural aralığın belirlenmesi sırasındaki istemsiz DP insidansı ile girişim deneme sayısı ve ponksiyon yapılan girişim bölgesinin anatomik yapısı arasında anlamlı ilişki saptanmıştır.
The incidence of accidental dural puncture during indentification of epidural space in parturients: Retrospective study
Objective: Whether there is a difference among accidental dural puncture (DP), number of attempts and anatomic structure during identification of epidural space in parturients was retrospectively investigated. Method: Identification technique of epidural space, number of attempts, puncture site and changing interspace, the anatomic structure of the patient at the puncture site (prominent, hardly palpable, not prominent), epidural needle type and diameter, incidence of dural and vessel puncture, side effects and complications and treatment approaches (conservative or epidural blood patch: EBP) were recorded in 972 parturients underwent epidural anesthesia for elective cesarean section. Results: Eighteen G epidural Tuohy needle and 20 G multihole epidural catheter were preferred in multipara (n=518) and primiparous (n= 454) parturients. Anatomic structure at the puncture site was found to be prominent in the majority of parturients (62.5%), while it was either hardly palpable (30.5%) or not prominent at all (7%). Puncture for epidural insertion was performed at L3-L4 (n=807), L2-L3 (n=148) and L4-L5 (n=17). Loss of resistance technique was used for identification of epidural space (99.7%) and it was successfully performed at the first attempt in 79.7% of the parturients. Distance between skin to epidural space was found to be 4.5 ± 0.7 cm. Accidental DP (n=12) and venous puncture during catheter insertion (n=35) were observed. While 10 subjects received conservative treatment, EBP was performed in two of them who were refractory to conservative treatment. No statistically significant difference was found between the incidence of accidental DP and number of attempts and anatomic structure of intervertebral spaces; and between number of attempts and anatomic structure of intervertebral spaces (p > 0.05). Conclusion: There was no significant difference among accidental DP, number of attempts and anatomical structure of the puncture site in the identification of the epidural space for elective cesarean sections.
