İdiyopatik erkek infertilitesinde probiyotiklerin yeri

İnfertilite; gebeliği önleyici bir yöntem kullanmamasına rağmen cinsel olarak aktif bir çiftin bir yıl içinde gebeliğe ulaşamaması şeklinde tanım- lanır. İnfertilite olgularının %30’a varan oranında erkeklerde spesifik bir etiyoloji belirlenemez ve idiyopatik infertilite olarak kabul edilir. Bu grupta klinik uygulamalarda ampirik ilaç tedavileri yaygın olarak kul- lanılmaktadır. Bu ilaçlar genelde antioksidan veya hormonal özellikte olabilmektedir. Probiyotikler, seminal mikrobiyom üzerine olan düzen- leyici etkileri, antioksidan ve antihiperlipidemik etkileri sayesinde özel- likle erkek idiyopatik infertilitesinde umut vaat eden destekleyici tedavi ajanları arasında düşünülebilir. Bu görüşün daha kuvvetli ifade edilebil- mesi için oldukça pahalı olmasına rağmen daha fazla sayıda deneysel ve klinik prospektif çalışmalar gereklidir.

Place of probiotics in idiopathic male infertility

Infertility is defined as the inability of a sexually active, non-contracepting couple to achieve pregnancy in one year. No specific etiology can be determined in men up to 30% of infertility cases and it is considered as idiopathic infertility. In this group, empirical drug therapies are widely used in clinical practice. These drugs may be antioxidant or hormonal. Probiotics can be considered as promising supportive agents especially in male idiopathic infertility due to its regulatory effects, antioxidant and anti hyperlipidemic effects on seminal microbiome. Although it is quite expensive more experimental and clinical prospective studies are needed to express this opinion stronger.
