Çocuklarda Barsak Perforasyonunun Nadir Görülen Bir Nedeni: Çoklu Mıknatıs Yutulması

Yabancı cisim yutulması çocukluk çağında sık rastlanan ev kazalarındandır. Çoklu mıknatıs yutulması diğer yabancı cisim yutmalarından farklı olarak mıknatısların çekim gücü nedeni ile barsak tıkanıklıklarına, perforasyonlarına ve sepsise varan ciddi komplikasyonlara yol açabilir. Herhangi bir başka bulgu ve başvuru yakınması olmadan çoklu mıknatıs yutulması öyküsü ile acil servise gelen iki, dört ve on üç yaşlarındaki üç çocuğun çekilen ayakta direkt karın grafisinde özofagus ve midede çoklu mıknatıslar izlendi. Üst gastrointestinal sistem endoskopisinde mıknatısların özofagus veya mideye yerleşip birbirlerini çekmeleri ve doku hasarı oluşturmalarının gözlenmesi nedeni ile laparotomi yardımlı çıkarıldı ve perforasyon alanları onarıldı. Ortalama 20 aylık takipte herhangi bir problem izlenmedi. Mıknatıs yutulması nedeni ile gelen çocukların radyolojik ve klinik değerlendirmeleri hızla yapılmalı ve erken girişim gereksinimi belirlenmelidir. Günümüzde yaygınlaşan mıknatıs içeren oyuncakların üzerine gereken uyarıların üreticiler tarafından konulması
ve korunaklı yerleştirilmesi, ailelere uygun eğitimin verilmesi olası tehlikenin engellenebilmesi için alınması gereken önlemlerdir.
An Unusual Cause of Intestinal Perforation in Children: Multiple Magnet Ingestion
Foreign body ingestion is one of the common home accidents. Multiple magnet ingestion can cause serious complications like intestinal obstruction, perforation and sepsis differently from the other foreign body ingestions because of their attraction capacity to each other. Three children with age of two, four and thirteen were admitted to emergency department with multiple magnet ingestion history, without any other symptom or complaints. In their abdominal X-rays, multiple magnets wereobserved in esophagus and stomach. Upper gastrointestinal endoscopy was performed and because of the attraction of the magnets which were settled different regions and created tissue damage, the magnets were removed by laparotomy and perforated areas were repaired. Patients’ follow up periods for twenty months were uneventful. A quick radiological and clinical evaluation must be done to determine the need of surgical attempt for the children who admitted to emergency department with the history of magnet ingestion. Producers should put warning labels on toys containing magnets which shows growing production rates nowadays and should pack them in sheltered boxes. Giving appropriate education to parents is one of the preventions to avoidpossible dangers.
