Ağrılı topuğun konservatif tedavisi ve ayak uzun arkı ile ilişkisi
Amaç: Ağrılı topukta uygulanan konservatif tedavinin sonuçlarını ve topuk ağrısı ile ayak uzun arkı arasındaki ilişkiyi değerlendirmek. Çalışma planı: Ayak tabanında ve topuğunda ağrı yakınması olan ve kontrol edilebilen 80 hastaya ait 101 ayak (62 kadın, 18 erkek, ortalama yaş 48) standart konservatif tedavi protokolüne göre tedavi edildi. Elli dokuz olguda tek taraf, 21 olguda ise çift taraf tutulumu vardı. Altmış sekiz ayakta tanı subkalkaneal ağrı sendromu, 21 ayakta plantar fasiitis, 10 ayakta ağrılı topuk yastığı ve iki ayakta lateral plantar sinirin ilk dalının sıkışması olarak kondu. Tedavi protokolü obez hastalarda kilo verilmesi, non-steroid anti-inflamatuar ilaç kullanılması, yumuşak tipte veya UCBL tipi tabanlık, ayakkabı-terlik alışkanlıklarının düzeltilmesi ve gerekirse lokal enjeksiyondan oluşmaktaydı. Tedaviye yanıt görsel ağrı skalası kullanılarak değerlendirildi. Olgular ortalama 13.6 ay (6-30 ay) süreyle izlendi. Sonuçlar: Uygulanan tedaviyle 61 olguda (%76.25) iyi 13 olguda orta (%16.25) ve altı olguda kötü (%7.5) sonuç alındı. Elli ayakta kalkaneal spur (%49.5) gözlendi; kontrol grubunda bu oran %23 idi. Radyolojik olarak yapılan ayak uzun ark yüksekliği ölçümlerinde hasta grup ile kontrol grubu arasında anlamlı farklılık saptanmadı; ancak hasta grupta topuk yağ yastığı kalınlığı artmış bulundu. Çıkarımlar: Ayakkabı alışkanlıklarını değiştiren ve/veya tabanlık kullanan kişilerde ağrının diğerlerine göre daha hızlı gerilediği; obezitenin, ağrılı topuk yastığının ve lateral plantar sinir ilk dalının sıkışmasının sonuçları olumsuz yönde etkilediği görüldü.
