Solunum yolu hastalığı olan çocuklarda fleksibl fiberoptik bronkoskopinin tanısal değeri
Amaç: Pediatrik yaş grubunda solunum problemleri nedeniyle fleksibl fiberoptik bronkoskopi (FFB) yapılan hastaların bulgularının geriye dönük analizi hedeflenmiştir.Gereç ve Yöntem: 2014-2016 tarihleri arasında, Mersin Kadın Doğum ve Çocuk Hastanesi Çocuk Göğüs Hastalıkları kliniğinde FFB uygulanan tüm çocuk hastalar çalışmaya dahil edilmiştir.Bulgular: Yaşları üç ay ile 16 yıl arasında değişen toplam 241 hastanın verileri değerlendirilmiştir. Hastalar <3 yaş (n=162, erkek cinsiyet: %63) ve ≥3 yaş (n=79, erkek cinsiyet:%54.4) olmak üzere iki gruba ayrılmıştır. Gruplar; demografik özellikleri, FFB endikasyonları, bulguları ve komplikasyonları açısından karşılaştırılmıştır. Üç yaş altı hastalarda en sık FFB endikasyonu stridor (%37) iken, üç yaş ve üstü hastalarda yabancı cisim şüphesi (%25.3) ön planda olmuştur. Patolojik FFB bulguları <3 yaş hastalarda en sık laringomalazi (%19.1) olarak saptanırken, ≥3 yaş hastalarda enfeksiyöz nedenler (%45.6) ön planda gözlenmiştir. Komplikasyon olarak geçici hipoksi <3 yaş hastalarımızda (%20.4), ≥3 yaş gruba (%10.1) göre istatistiksel olarak anlamlı olan şekilde fazla gözlenmiştir.Sonuç: Pediatrik yaş grubunda FFB işlemi solunum yolu patolojilerini tanımlamada etkin roloynayan güvenilir bir tanı yöntemidir.
Diagnostic value of flexible fiberoptic bronchoscopy in pediatric patients with respiratory problems
Purpose: We aimed to analyze the findings of our pediatric patients who underwent flexible fiberoptic bronchoscopy (FFB) due to respiratory problems retrospectively.Materials and Methods: The medical records of all children who underwent FFB procedure at Mersin Women and Children’s Hospital Pediatric Pulmonology Section between 2014-2016 were included in the study.Results: A total of 241 patients aged between 3 months and 16 years were evaluated. The patients were divided into two groups according to their age (<3 years of age; n=162, male gender: 63.0% and ≥3 years of age age: n=79, male gender: 54.4%). Then, these two groups were compared among their demographic features, FFB indications, FFB findings and complications. The most frequent indication for FFB in patients <3 years of age was stridor (37%), and patients ≥3 years of age was suspicious foreign body aspiration (25.3%). The most common pathologic finding was laryngomalacia (19.1%) in patients <3 years of age and infection (45.6%) in patients ≥3 years of age. Transient hypoxemia was observed more prominent in patients <3 years of age than ≥3 years of age (20.4% versus 10.1%).Conclusion: Pediatric FFB appears to be a safe and valuable procedure in the diagnosis of respiratory diseases in pediatric patients.
