Genç kadın olguda spontan intraserebral hemoraji migren? EKT ??

Iskomik serebrovasküler hastalıklarda özellikle auralı migren bir etyolojik neden olarak yer almaktadır. Çok ender olarak migren ataklarına bağlı intraserebral hemoraji de gelişebilir. Bu yazıda sunulan 34 yaşında bayan hasta, 10 yıldır, ayda 3-4 kez aurasız migren atakları tanımlamaktadır. Kendisine obsesif kompülsif bozukluk nedeniyle izlendiği psikiyatri kliniğinde, sonuncusu, yakınmalarından bir hafta önce olmak üzere yedi kez elektrokonvulsif tedavi uygulanmıştır. Öncekilere benzer bir migren atağından 8 saat sonra aniden ortaya çıkan sol hemiparazi sendromu nedeniyle kliniğimize yatırılmış ve aynı gün çekilen bilgisayarlı beyin tomografisinde sağ parietal bölgede, lober hematom saptanmıştır. Bu bağlamda, olgumuzdaki intraserebral hemoraji gelişiminde migren ve/veya EKT uygulamasının olası etkileri tartışılmıştır.

Spontaneous intracerebral hemorrhage in a adult young female: Migraine? ECT??

Migraine with aura is considered as an etiologic factor in ischemic cerebrovascular disorders. While, intracerebral hemorrhage occurs in rare instances during or after migraine attack. A thirty-four year-old female patient has migraine without aura for ten years with a frequency of 3-4 attacks monthly. She underwent electroconvulsive therapy for seven times in the psychiatry department because of obsessive-compulsive disorder a week ago. Following a migraine attack a left hemiparesis syndrome occurred, and computed tomography, which was performed at the same day, revealed a right parietal lobar hematoma. In this paper, lobar hemorrhage in a young patient and its etiologic causes in relation to migraine and/or electroconvulsive therapy were discussed.
