FARKLI EVRELERDE DİZ OSTEOARTRİTİ OLAN HASTALARDA AYAK VE AYAK BİLEĞİ DEĞERLENDİRİLMESİ

Amaç Ayak ve ayak bileği profili alt ekstremitenin mekanik dizilimini ve dinamik fonksiyonunu, özellikle diz ekle mi mekaniğini değiştirmektedir. Farklı derecelerde diz osteoartriti (OA) olan hastalarda; ayak ve ayak bileği duruşundaki değişiklikleri belirlemek tedavi yaklaşım ları açısından oldukça önemlidir. Bu çalışmanın ama cı, Kellgren-Lawrence (K-L) ’a göre farklı derecelerde medial kompartman diz OA olan hastaların ayak ve ayak bileği profilindeki değişiklikleri ve etkilenimleri değerlendirmekti. Gereç ve Yöntem Bu kesitsel çalışmaya dahil olan tüm hastaların OA ev resi, ağırlık taşıyarak çekilen ön-arka ve yan diz radyog rafilerine göre K-L derecelendirme sistemi kullanılarak belirlendi. Western Ontario ve Mc Master Üniversitesi Osteoartrit İndeksi, hastaların osteoartrite bağlı ağrı larını ve fonksiyonel durumlarını değerlendirmek için kullanıldı. Ayak ve ayak bileği profilinin ve fonksiyonel liklerinin belirlenmesinde Ayak Postür İndeksi ve Ame rikan Ortopedik Ayak ve Ayak Bileği Derneği Ayak Bile ği (AOFAS)-Arka Ayak ve Orta Ayak ölçekleri kullanıldı. Hastaların ayak ağrısı şiddeti görsel analog skalası, ayağa özgü sağlıkla ilişkili yaşam kalitesi ise Manches ter-Oxford Ayak Anketi kullanılarak değerlendirildi. Bulgular Çalışmamıza yaş ortalaması 57,12±12,2 yıl olan top lam 112 hasta dahil edildi. Diz OA şiddetine göre sı nıflandırılan hastaların %24’ü Evre 0-1, %24’ü Evre 2, %29’u Evre 3 ve %23’ü Evre 4 grubunu oluşturdu. Hastaların ayak değerlendirmesinde kullanılan APİ sonuçlarına göre hastaların %18,4’ü supinasyonda, %42,5’i nötralde ve %39,1’i pronasyonda olarak de ğerlendirildi. FOOT AND ANKLE ASSESSMENT IN PATIENTS WITH DIFFERENT GRADES OF KNEE OSTEOARTHRITIS

Objective The foot and ankle profile change the mechanical alignment and dynamic function of the lower limbs, especially the knee joint mechanics. Identifying chan ges in foot and ankle postures is very important in ter ms of treatment approaches in patients with different degrees of knee osteoarthritis (OA). The aim of this study was to assess the changes and effects on the foot and ankle profile of patients with different grades of medial compartment knee OA as a Kellgren-Law rence (K-L). Materials and Methods All patients included in this cross-sectional study were divided into four groups for medial compart ment OA severity by using the K-L grading system of knee OA on weight bearing lateral and anterior-pos terior knee radiographies. The Western Ontario and Mc Master University Osteoarthritis Index (WOMAC) questionnaire was performed to assess the pain and the functional status of the patients. Structural foot profile was assessed by the Foot Posture Index (FPI) system and American Orthopaedic Foot and Ankle Society (AOFAS) Ankle–Hindfoot and Midfoot scales. Also, the severity of foot pain of the patients was eva luated with a visual analogue scale and foot-specific health related quality of life of the patients was me asured using the Manchester-Oxford Foot Question naire (MOFQ). Results The study included 112 patients with mean age 57.12±12.2 years. In terms of severity of knee OA groups; percentages for grade 0-1,2,3 and 4 were 24%, 24%, 29%, 23% respectively.
