Sporcularda Paget Schroetter Sendromu (Primer Efor Trombozu)

Aksiller-subklavian ven trombozu ya da efor trombozu olarak da bilinen Paget-Schroetter sendromu sağlıklı, genç bireylerde özellikle üst ekstremitelerin tekrarlayan abduksiyonu ile karakterize zorlu hareketleri sonrasında, sıklıkla dominant kolda gelişen bir durumdur. Aşırı efor öyküsü ve anatomik özelliklerin yanı sıra hastada altta yatan koagulasyon problemleri bulunması Paget-Schroetter sendromu gelişme olasılığını artırır. Farklı yayınlarda ve olgu sunumlarında çeşitli tedavi yöntemlerinden bahsedilse de henüz tedaviyle ilgili görüş birliği sağlanamamıştır. Tedavi edilmeyen ya da yetersiz tedavi edilen hastalarda gelişebilecek pulmoner emboli, serebral emboli ve post-trombotik sendrom gibi ciddi komplikasyonların yanı sıra sporcunun aktiviteye dönüşünün engellenmesi söz konusu olabileceğinden PagetSchroetter sendromu ve kliniği iyi bilinmesi mümkün olan en kısa sürede tedaviye başlanması açısından önemlidir. Bu derleme yazısında Paget-Schroetter sendromunun patogenezi, oluşumunu kolaylaştıran faktörler, kliniği, tanı yöntemleri, tedavi seçenekleri ve erken tanının tedavi açısından önemine dikkat çekmek amaçlanmıştır.

Paget Schroetter Syndrome in Athletes (Primary Effort Thrombosis)

Paget-Schroetter syndrome, also known as axillary-subclavian venous thrombosis or effort thrombosis, is condition that develops frequently in healthy, young individuals, especially after repetitive abduction movement of the dominant upper extremity. Anatomical or hematological factors play role in the pathogenesis of Paget-Schroetter syndrome. Although various treatment modalities have been mentioned in different reports, there is no consensus on treatment. Early diagnosis of Paget-Schroetter syndrome is crucial to start treatment as soon as possible, as serious complications such as pulmonary embolism, cerebral embolism and post-thrombotic syndrome may develop in untreated or inadequately treated patients. In this review article, we aimed to draw attention to the importance of Paget-Schroetter Syndrome in terms of pathogenesis, factors facilitating the formation, clinics, diagnostic methods, treatment options and early diagnosis.
