İmmün sistem yetersiz kalırsa organizma basillere karşı spesifik koruma meydana getiremez. Tüberküloz basilinin endo ve ekzotoksini yoktur. Bu hastalarda genellikle miliyer tüberküloz görülür ve koch basillerinin hematojen yayımı sonucu her organda çok sayıda küçük tüberküloz granülomları saptanır.
