SUMMARY A RARE COMPLICATION OF THE ADULT POLYCYSTIC KIDNEY DISEASE: OBSTRUCTIVE NEPHROPATHY OF THE PARAPELVIC CYST AND ITS PERCUTANEOUS TOTAL ABLATION Autosomal dominant polycystic kidney disease (ADPKD) is an inherited disorder progress slowly by many symptoms and cause to death with its severe complications such as uremia, hypertension and renal calculi at an average age of 50. Total enlargement of the kidneys can be seen in these patients but the obstruction of urinary tract due to a parapelvis enlarged cyst is not a common complication. In our case, the uncommon complication of ADPKD was demonstrated by the help of radiological methods. Furthermore, the recovery of hydronephrotic kidney was shown after the US-Guided percutaneous ablation of the cyst by a sclerosing agent. ÖZET Erişkin tipi polikistik böbrek hastalığı (EPBH) otozomal geçiş gösteren ve neden olduğu böbrek taşı, üremi ve hiper¬tansiyon gibi komplikasyonlarıyla hastaları 50 yaş dolayında ölüme götüren herediter bir hastalıktır. Böbrek boyutlarının ileri derecede büyüyebildiği bu hastalıkta, parankimal kist oluşumlara rağmen parapelvik yerleşimli kistlerin renal pelvise basacak şekilde büyüyerek obstrüktif bir üropati yapmaları beklenen bir komplikasyon değildir. Olgumuzda bu tür bir komplikasyona yol açan parapelvik kist radyolojik yöntemlerle ortaya konulmuş, ayrıca kistin US rehberliğinde skatrizasyonu sağlanarak obstrüksiyon nedeni ortadan kaldırılmıştır.
