Primer inguinal hernilerde Lichtenstein herniorafi

Seksenli yıllardan sonra kasık fıtığı onarımında prostetik mesh kullanımı giderek popülarize olmuş ve yaygın olarak uygulama alanına girmiştir. Postoperatif gerginlik olmaması ve lokal anestezi ile uygulanabilirliği ile Lichtenstein Herniorafi tekniği günümüzde en sık uygulanan onarım tekniklerinden biri olup, prostetik mesh yaygın kullanılırlığında bir çığır açmıştır. Kliniğimizde tekniğin uygulandığı 200 primer inguinal hernili hastada retrospektif değerlendirmede ortalama izlem süresi 20 aydır. % 1 rekürrens, oral analjeziklere cevap veren minimal postoperatif ağrı, % 1 morbidite gözlenmiş ve literatür verileri ile karşılaştırılmıştır. Düşük rekürrens, erken mobilizasyon, kısa hospitalizasyon, günlük aktiviteye erken dönüş ve iş gücü kaybının azaltılması, yüksek postoperatif hasta konforu, düşük ekonomik maliyet gibi modern cerrahi gereklerinin sağlandığı ve primer inguinal hernilerde güvenle uygulanabileceği gözlenmiştir.

Lichtenstein herniorapy in primary inguinal hernia

Since the 1980's, a dramatic progress has been made in the evolution of groin hernia surgery, especially by the increasing use of prosthetic mesh. The Lichtenstein tension-free mesh repair has opened a new era in groin hernia without the risk of serious morbidity. 200 primary hernia repair cases are reviewed and results are reported comparing with literature data in this study. Minimal recurrence rate (%1), early mobilization, short hospital stay, early ambulation and decreased time-off work have been obtained and the results are similar with literature. As a result, Lichtenstein tension-free herniorraphy technique is preferred as a first choise in groin hernia repair.
