Ortognatik cerrahi hastaları için bireysel olarak hazırlanan beslenme programlarının Hastaların ameliyat sonrası vücut bileşimlerine ve beslenme durumlarına etkisi

Amaç: Maksillofasiyal cerrahi sonrasında enerji ihtiyacı artmaktadır. Aynı zamanda hastaların uzun süre sıvı ve yumuşak besin ile beslenmesi gerekebilmektedir. Artan günlük enerji ihtiyacının yeterince karşılanamaması malnutrisyonla sonuçlanabilir. Biz bu çalışmada ortognatik cerrahi sonrasında hastalara bireysel olarak hazırlanan beslenme programlarının hastaların günlük enerji ve besin öğesi ihtiyacını karşılamadaki etkinliğini belirlemeyi amaçladık. Yöntem: On dört ortognatik cerrahi hastası çalışmaya dahil edildi. Hastaların nutrisyonel parametreleri ameliyat öncesi, ameliyat sonrası 7. gün ve 21. gün değerlendirildi. Değerlendirme biyoelektrik impedans yöntemi ve laboratuvar parametreleri (albümin, prealbumin, C-Reaktif Protein ve lenfosit sayımı) kullanılarak yapıldı. Ameliyat öncesinde yapılan vücut bileşimi analizinde hesaplanan bazal metabolizma hızı kullanılarak ameliyat sonrası tahmini enerji ihtiyacı hesaplandı ve bu ihtiyaca yönelik uygun beslenme programı verilerek hastalar takip edildi. Elde edilen veriler Freidman testi ve Bonferroni düzeltmeli Mann-Whitney U testi kullanılarak analiz edildi. Bulgular: Hastaların beden kütle indeksi açısından ardışık ölçümler arasında anlamlı bir farklılık bulunmadı. Postoperatif 7. günde serum malnutrisyon parametrelerinden prealbumin seviyesinde ortalama 4.7mg/dL, albumin seviyesinde ortalama 0.9mg/dL düşüş, C-reaktif proteinde 38.1mg/L artış tespit edildi. Diğer serum parametreleri açısından ardışık ölçümlerde anlamlı fark tespit edilmedi. Vücut bileşimi analizinde vücut yağ ağırlığı, iskelet kas ağırlığı, hücre içi ve hücre dışı su dağılımı bakımından ardışık ölçümler arasında anlamlı fark bulunmadı. Sonuç: Yapılan az sayıda benzer çalışmada ortognatik cerrahi sonrasında belirgin ağırlık kaybı bildirilmiş olmasına karşın bu çalışmada hastaların vücut kitle indeksi, vücut yağ oranı, iskelet kası ağırlığı ve sıvı dengesi ile serum nutrisyon parametrelerinde belirgin değişiklik görülmedi. Hastaların postoperatif beslenmesinde uygulanan bireysel beslenme programının ortognatik cerrahi hastalarının vücut bileşimi ve beslenme durumunun korunmasında etkin bir yöntem olduğu sonucuna ulaşıldı.

The effect of individualized dietary programs on postoperative body composition and nutritional status of the orthognathic surgery patients

Aim: Daily energy requirements increase after maxillofacial surgery. Along with this, patients experience nutritional problems because of long-time fluid or soft-diet feeding after a jaw surgery. Both of them can be resulted with malnutrition. In this study, we aimed to study the effect of individualized nutrition programs on nutritional parameters in patients having an orthognathic surgery. Methods: A total of 14 patients who underwent orthognathic surgery were included in this study. Patients were evaluated just before the surgery and at postoperative days 7 and 21. Nutritional status was assessed by using the bioelectrical impedance method and also laboratory parameters (albumin, prealbumin, C-Reactive Protein and lymphocyte count). A diet schedule and calculated caloric requirement plan was prepared for each patient according to body composition analysis results. Measurements were analyzed using the Freidman test and Bonferroni-corrected Mann-Whitney U test. Results: There was no significant difference between preoperative and postoperative body mass index values. A significant decrease in prealbumin (mean 4.7mg/dL) and albumin levels (mean 0.9mg/dL) and increase in C- reactive protein (mean: 38.1mg/L) level were seen at day 7. According to body composition analysis no significant differences were found in the means of fat weight, skeletal muscle weight and intracellular-extracellular water distribution at postoperative days 7 and 21. Conclusion: Despite of other few reports regarding significant decrease in body weight after orthognathic surgery, no significant differences in body weight, fat, muscle and mineral composition of body and nutritional parameters in blood were seen in our study. We thought that individualized nutritional programs were effective to meet the nutritional needs of orthognathic surgery patients
