Kronik hepatit B infeksiyonunda serum prokalsitonin seviyeleri

Giriş: Kalsitonin hormonunun prekürsörü olan prokalsitonin (PCT) düzeyinin, bakteriyel infeksiyonlar ve sepsisli hastalarda arttığı gösterilmiştir. Bu çalışmada kronik hepatit B (KHB) tanısı almış olan hastalarda serum PCT düzeylerinin ve karaciğer histopatolojisi ile PCT arasındaki korelasyonun değerlendirilmesi amaçlanmıştır. Materyal ve Metod: Çalışma, Ocak-Aralık 2010 tarihleri arasında takip edilen 30 KHB hastası ile çalışmaya gönüllü olarak katılan 30 sağlıklı erişkinde yapıldı. Hastalardan alınan karaciğer biyopsi örneklerinin histopatolojik değerlendirmesi modifiye Knodell sistemine göre yapıldı. PCT seviyesi Enzyme-Linked Immunosorbent Assay (Ray Bio® Human Procalcitonin ELISA Kit, Norcross GA, USA) ile ölçüldü. Verilerin analizinde ki-kare testi ile T testi ve hasta grubunda değişkenlerin korelasyon analizinde Kendall tau b korelasyon testi kullanıldı. Bulgular: Çalışmaya, yaşları 15-65 yıl arasında değişen, %61.7 (n= 37)'si erkek olmak üzere toplam 60 kişi dahil edildi. Tüm katılımcıların yaş ortalaması 29.0 ± 12.0 yıl idi. Hastalarda ortalama PCT seviyesi (0.63 ± 0.12 ng/mL), kontrol grubuna göre (0.37 ± 0.08 ng/mL) yüksek bulundu (p< 0.001). Hastaların karaciğer biyopsilerinin histolojik aktivite indeksi ve fibrozisi ile PCT seviyeleri arasında korelasyon saptanmadı (sırasıyla t= -0.007, p= 0.957, t= 0.153, p= 0.282). Sonuç: Çalışmada, KHB hastalarında PCT seviyeleri sağlıklı kontrol grubundan daha yüksek tespit edilmiştir. Artan inflamasyon ve fibrozis derecesi ile PCT seviyelerinde artış saptanmamıştır. Bu konuda fazla sayıda hastayla yapılacak çalışmalara ihtiyaç vardır.

Serum procalcitonin levels in chronic hepatitis B infection

Introduction: Procalcitonin (PCT), pro-hormone of calcitonin, was found to be increased in bacterial infections and sepsis. The aim of this study was to evaluate serum PCT levels in patients with chronic hepatitis B (CHB) and the correlation between the liver histopathology and PCT levels. Materials and Methods: The study was conducted between January-December 2010. Thirty patients and 30 healthy controls were included in the study. Histopathological evaluation of liver biopsy specimens from patients was performed according to the modified Knodell system. PCT levels were measured by enzyme-linked immunosorbent assay (Ray Bio® Human Procalcitonin ELISA Kit, Norcross, GA, USA). Chi-square test and T test were used in the analysis of the data, and Kendall tau b correlation test was used in the correlation analysis of the variables in the patient group. Results: A total of 60 participants were included in the study. Of them, 61.7% (n= 37) were male, and the ages ranged from 15 to 65 years. The mean age of the participants was 29.0 ± 12.0 years. The mean PCT levels of patients (0.63 ± 0.12 ng/mL) were higher than in the control group (0.37 ± 0.08 ng/mL) (p< 0.001). No correlation was found between the histological activity index and the degree of fibrosis in liver biopsies of patients and PCT levels (t= -0.007, p= 0.957; t= 0.153, p= 0.282, respectively). Conclusion: This study showed that serum PCT levels were found higher in CHB patients than in the healthy control group. PCT levels did not increase with increasing degree of inflammation and fibrosis. Further large-scale studies are needed to verify this conclusion.
