Genel Anestezide Kullanılan Propofolün Başlangıç Dozunun Bulanık Mantık ile Tahmini
Bu çalışmada, en yaygın kullanılan genel anestezi ilaçlarından propofolün, başlangıç dozunun hesaplanmasında bulanık mantık kullanılmıştır. Çalışma bulanık mantık kullanarak girişte hastanın yaş, vücut kitle indeksi (VKİ) ve Amerikan Anestezistler Derneği’nin (AAD) belirlediği hasta sınıfını alarak çıkışta hastaya verilmesi gereken propofol başlangıç dozunu (PBD) kg/mg cinsinden vermektedir. Çalışmada başlangıç dozunun, premedikasyon yapılmadığı varsayılarak ilave ilaç kullanılmadan hesaplanması esas alınmıştır. Birden fazla anestezi uzmanının yardımı alınarak kural tabanı oluşturulmuştur. Çalışma sonunda bulunan propofol doz değerleri uzman doktorların değerlerine oldukça yakın çıkmıştır. Bu sebeple bu çalışmada kullanılan bulanık mantık tahmincisi anestezi uzmanlarına destek sağlayıcı olarak değerlendirilebilir.
Estimation of Initial Dose of Propofol Used in General Anesthesia with Fuzzy Logic
Fuzzy logic is the type of logic that defines correctness, error and rating of classical logic. It assists specialists in healthcare sector in solving various problems. Initial doses of anesthetic medication prescriptions are given by specialists based on age, weight, height, previous surgical operations and comorbidities. In this study, fuzzy logic is used to estimate propofol initial doses (PID) from common anesthesia medications. It uses fuzzy logic to previse propofol initial doses estimated in kg/mg for general anesthesia depending on age, body mass index (BMI), the American Society of Anesthesiologists’ (ASA) classification of diseases and identifying disease groups. Initial doses are to be calculated without additional medication, assuming no premedication is applied. The rule base is established through assistance of anesthesiologists. Propofol dose values found with fuzzy logic are similar to the values of physicians. Therefore, the fuzzy logic estimator used in this study can support anesthesia specialists.
