Postpartum Depresyon ile Emzirme Arasındaki İlişkinin Değerlendirilmesi
Amaç: Yapılan çalışmalar postpartum depresyon ve emzirme durumu arasındaki ilişkinin çift yönlüolabileceğini; postpartum depresyonun emzirme oranlarını azaltabileceğini ve emzirme güçlüklerinin dePPD riskini artırabileceğini göstermektedir. Bu çalışma ile 2 yaş ve altı bebeği olan annelerdepostpartum depresyon sıklığını, emzirme öz yeterlilik düzeylerini ve ilişkili faktörleri ve postpartumdepresyonun emzirme üzerine olan etkilerini ayrıntılı olarak araştırmayı amaçladık.Materyal ve Metot: Bu tanımlayıcı tipte planlanan çalışmaya; 2 hafta ile 24 ay arası bebeği olan 511 annekatılmıştır. Veriler; annelerin sosyo‐demografik özellikleri, emzirme durumu ve postpartum depresyoniçin risk faktörlerine yönelik hazırlanan anket formu; Edinburgh Postpartum Depresyon Ölçeği;Emzirme Öz Yeterlilik Ölçeği kullanılarak internet ortamında hazırlanan anket formu ile 01.01.2018‐15.01.2018 tarihleri arasında toplanmıştır.Bulgular: Tüm annelerin %19.90’ında postpartum depresyon saptanmıştır. Kadınların EdinburghPostpartum Depresyon Ölçeği puan ortalaması 11,68±5,75 (min=0, maks=27), Emzirme Öz YeterlilikÖlçeği puan ortalaması 49,50±10,13 (min=21, maks=70)’dür. Çalışmamızda annelerin Emzirme ÖzYeterlilik Ölçeği ile Edinburgh Postpartum Depresyon Ölçeği puanları arasında negatif bir korelasyonbelirlenmiştir. İlk yarım saatte emzirmeye başlama, sık emzirme durumları ile postpartum depresyonarasında istatistiksel olarak anlamlı bir ilişki bulunmamıştır; ancak ve emzirme süresi ile postpartumdepresyon arasında istatistiksel olarak anlamlı bir ilişki bulunmuştur.Sonuç: Emzirme ve postpartum depresyon arasındaki ilişkinin yönü nasıl olursa olsun bu iki faktörünbirbirini etkilediği kaçınılmaz bir gerçektir. Özellikle doğum öncesi ve doğum sonrası anne ile en çokkarşılaşan sağlık çalışanları olan aile hekimleri ve aile sağlığı elemanlarının emzirme danışmanlığıvermek ve annenin psikiyatrik değerlendirmesini yapmak gibi kritik görevleri bulunmaktadır.
The Evaluation of the Relationship Between Postpartum Depression and Breastfeeding
Objectives: Recent studies show that the relationship between postpartum depression and breastfeeding status may be bi‐directional; postpartum depression can reduce breastfeeding rates and breastfeeding difficulties can increase postpartum depression risk. In this study, we aimed to investigate the frequency of postpartum depression, breast self‐efficacy levels and related factors and the effects of PPD on breastfeeding in mothers of 2 years and younger. Materials and Methods: 511 mothers with 2 weeks‐24 months old babies were enrolled to this descriptive type planned study. Data was collected between 01.01.2018 and 15.01.2018 with the questionnaire form prepared on the internet using a questionnaire prepared for the risk factors for maternal socio‐demographic characteristics, breastfeeding status and postpartum depression; Edinburgh Postpartum Depression Scale; Breastfeeding Self‐Sufficiency Scale. Results: Postpartum depression was detected in 19.90% of all mothers. The average Edinburgh Postpartum Depression Scale scores of the women was 11.68 ± 5.75 (min = 0, max = 27) and the mean score of the Breastfeeding Self‐Sufficiency Scale was 49.50 ± 10.13 (min = 21, max = 70). In our study, there was a negative correlation between the Breastfeeding Self‐Sufficiency Scale scores and the Edinburgh Postpartum Depression Scale scores of the mothers. No statistically significant relationship was found between postpartum depression and frequent breastfeeding in the first half hour; but there was a statistically significant relationship between the duration of breastfeeding and postpartum depression. Conclusion: Regardless of the direction of the relationship between breastfeeding and postpartum depression, it is inevitable that these two factors influence each other and particularly for family physicians and family health workers, who are the health professionals most frequently encountering prenatal and postnatal mothers, there is a great deal of workload in this area.
