Down Sendromu İçin Gebelere Yapılan Prenatal Tarama ve Tanı Testlerinin Sonuçları; Maliyet Analizi
Amaç: Down Sendromu ( Trizomi 21), en yaygın görülen kalıtsal sendrom olup, yaklaşık her 800 ile1000 canlı doğumda bir karşımıza çıkmaktadır. Ölümcül olmayan trizomi olduğu için, genetik taramave tanı test protokollerinin çoğunun odağındadır. Çalışmamızın amacı, hastanemizde yapılan prenataltarama ve tanı testlerinin maliyet değerlendirmesini yapmak ve bu harcamalar karşılığında canlı doğanDown Sendromu olgularının sayısını belirlemektir.Materyal ve Metot: Çalışmada, SBÜ Zekai Tahir Burak Kadın Sağlığı Eğitim ve AraştırmaHastanesi’nde Mayıs‐Temmuz 2017 tarihleri arasında antenatal tarama sonuçlarına göre tanı testleriuygulanmış ve gebeliği sonuçlanmış (doğum, abort, terminasyon) 2300 gebenin arşiv dosyalarıtaranmıştır. Her prosedürün maliyet değeri hesaplanarak prenatal Down Sendromu tarama ve tanısıiçin toplam maliyet değeri elde edilmiştir. Bulgular: Hastanemiz antenatal polikliniklerinde takip olmuş ve gebeliği sonuçlanmış 2300 gebedenprenatal Down Sendromu taraması olan 1096 gebenin sonuçları değerlendirilmiştir. Prenatal tanı içininvaziv prosedür uygulama endikasyonu olanların genetik tanı için 19’una (% 1,73) CVS ve 50’sine (%4,56) amniyosentez yapılmıştır. Test sonuçlarına göre 8 (% 0,72) gebeye Trisomi 21 tanısı konulmuştur.Bunlardan dört gebeye (1 CVS ve 3 amniyosentez sonrası) gebelik terminasyonu yapılmış, iki gebelik(CVS sonrası) spontan düşük ile, bir gebelik preterm ölü doğum ile sonuçlanmış, bir gebe ise canlıdoğum yapmıştır. Aynı dönem içinde tarama testi yüksek riskli çıkmış, amniosentez yaptırmayı kabuletmemiş bir gebe de Down Sendromlu canlı bebek dünyaya getirmiştir. Sonuç olarak tarama yapılansadece iki bebek (% 0,18) miadında canlı doğmuştur. Maliyet değerlendirmesinde bu gebelerin sadeceikili tarama testi yaptıranlara toplamda 11.856,00 TL, sadece üçlü tarama testi yaptıranlara 14.169,30 TL,hem ikili hem üçlü tarama testi yaptıranlara 75.738,30 TL para harcanmıştır. Amniyosentez yöntemi ilegenetik tanı koyulan gebelere 16.810,00 TL, CVS yöntemi ile genetik tanı koyulan hastalara 7.653,20 TLpara harcanmıştır. Toplamda 126.226,80 TL harcayarak, 8 gebeye Down Sendromu tanısı koyulmuştur. Sonuç: Down Sendromu konusunda aileleri detaylı bilgilendirerek prenatal tarama testleri daha efektifkullanılabilir. Böylece tahliye istemi olmayan bireylere yapılan harcamalar canlı doğan DownSendromlu olguların sağlık sorunlarının çözülmesi ve onların hayat kalitesini arttırarak toplumakazandırılması yönünde kullanılabilir. Optimal tıbbi ve sosyal destek sağlanırsa, özel değerlendirmelerve incelemeler yaparak Down Sendromlu bireylerin yaşam kaliteleri önemli ölçüde arttırılabilir vetopluma önemli katkılar sağlanabilir.
Results of Prenatal Down Syndrome Screening and Diagnostic Tests for Pregnants; Cost Analysis
Objectives: Down Syndrome is the most common hereditary syndrome and presents approximately one in every 800 to 1000 live births. Because Down Syndrome is the most common non‐fatal trisomy, It is on the focus of the most genetic screening and diagnostic testing protocols. The aim of our study is to evaluate the cost of prenatal screening and diagnostic tests performed in our hospital and to find out the number of live ‐born babies with Down syndrome.Materials and Methods: The study was conducted between May‐July 2017 SBU Zekai Tahir Burak Women’s Health Education and Research Hospital. The recorts of 2300 pregnant women were screened. The cost of down syndrome screening and diagnosis was calculated.Results: Results of 1096 pregnant women with prenatal Down syndrome screening from 2300 pregnants who were followed up in antenatal outpatient clinics of our hospital were evaluated. 19(1,73%) CVS and 50(4,56%) amniocentesis were performed as an invasive procedure for the genetic diagnosis of the pregnants. According to the test results, 8(%0,72) pregnant women were diagnosed as Trisomy 21. Pregnancy outcomes were like these; pregnancy termination applicated to 4 pregnancies (1 CVS and 3 Amniocentesis cases), 2 pregnancies (post‐CVS) aborted spontaneously, 1 pregnancy resulted in preterm stillbirth and one pregnant live birth. During the same time period, one pregnant with high risk screening test result didn't accept amniocentesis and had a live baby with Down Syndrome. As a result, only 2 (0,18%) infants with Down syndrome were live birth. We spent a total of 11.856,00 TL for those who had only double screening test done, 14.169,30 TL for those who had only triple screening test done, 75.738,30 TL for those had both double and triple screening tests done, 16.810,00 TL for pregnants who were genetically diagnosed by amniocentesis and 7.653,20 TL for pregnants who were genetically diagnosed by CVS method. Only eight Down Syndrome cases diagnosed by spending a total of 126.226,80 TL.Conclusion: As a result of the more effective use of prenatal tests with accurate councelling about Down Syndrome, expences on pregnants who reject pregnancy termination can be used to solve the health problems of live born children with Down Syndrome and to bring them to community by increasing their quality of life. If optimal medical and social support is provided, the life quality of individuals with Down's syndrome can be significantly improved and significant contributions can be made to the community by making special assessments and examinations.
