Mesane Tümörünün Transüretral Rezeksiyonunda Ultrason Eşliğinde Yapılan Obturator Sinir Bloğunun Etkinliği

Amaç: Bu çalışma kliniğimizde mesane tümörü nedeniyle Transüretral Rezeksiyon TUR geçiren hastalarda, spinal anestezi ile beraber uyguladığımız ultrason eşliğinde obturator bloğun etkinliğini araştırmayı amaçlamaktadır.Gereç ve Yöntemler: Etik kurulu onayı alındıktan sonra 2014 Mart- 2015 Aralık arası TUR yapılmış olan hastaların anestezi kayıtları incelendi. Kayıtlardan hastaların yaş, cinsiyet, operasyon süresi, anestezi yöntemi, obturator sinir bloğu uygulanıp uygulanmadığı, adduktor refleks ve komplikasyonlar incelendi.Bulgular: 37 hastanın spinal anestezi ile TUR geçirdi ve bu hastaların 20 tanesine ek olarak obturator sinir blokajı uygulandı. Hastaların yaş ortalaması 67,95 ±13,5 yıldı. Obturator sinir blokajı yapılan hastalardan sadece bilateral blok yapılan bir hastada obturator refleks görüldü. Blok yapılmayan ve sadece spinal anestezi uygulanan 17 hastanın üçünde obturator refleks gözlendi. Hastaların hiçbirinde diğer komplikasyonlar görülmedi.Sonuç: Transüretral rezeksiyon yapılan hastalara obturator refleksi önlemek için spinal anesteziye eklenen obturator sinir bloğu başarılı bir anestezi sağlar ve komplikasyonları azaltır. Ultrason kullanımı obturator bloğun başarı oranını artırmaktadır

Effectiveness of Ultrasound Guided Obturator Nerve Block in Transurethral Resection of Bladder Tumor

Objective: This study aimed to evaluate the efficacy of ultrasound-guided obturator nerve block, administered with spinal anesthesia, in patients undergoing transurethral resection TUR of bladder tumors.Material and Methods: After receiving the approval of the ethics committee BAEK 2016/0701 , the records of the patients who underwent TUR between March 2014 and December 2015 were evaluated. Patient data including age, gender, duration of operation, the method of anesthesia administered, administration of obturator nerve block, adductor reflex, and the complications were observed.Results: 37 patients underwent TUR with spinal anesthesia and obturator nerve blockade was performed in addition to 20 of these patients. The mean age of the patients was 67.95±13.5 years. Of the patients in whom obturator nerve block was performed, the obturator reflex occurred only in one patient who received a bilateral block. Of the 17 patients who were administered only spinal anesthesia without an obturator nerve block, the obturator reflex occurred in three patients. No other complications were observed in the patients.Conclusion: Obturator nerve block added to spinal anesthesia to prevent obturator reflex in patients undergoing transurethral resection provides successful anesthesia and reduces complications Utilization of ultrasonography increases the success rates of the obturator nerve block
