Bir Çocuk ve Ergen Psikiyatrisi Kliniğine Başvuran Travma Sonrası Stres Bozukluğu Olgularının Sosyodemografik ve Tanısal Özellikleri

Amaç: Travma sonrası stres bozukluğu TSSB , çocuk ve ergenlerin yaşamlarını etkileyen önemli bir psikiyatrik hastalıktır. Dolayısıyla bu çalışmanın amacı, bir çocuk ve ergen psikiyatrisi bölümüne başvuran TSSB'li hastaların sosyodemografik ve tanısal özelliklerini ortaya koymaktır.Gereç ve Yöntemler: Çalışmaya katılan çocuk ve ergenlere araştırmacılar tarafından oluşturulan sosyodemografik verilerin ve yaşanılan travmatik olayla ilgili soruların olduğu bir form doldurulmuştur. Ayrıca psikiyatrik tanılar için yarı yapılandırılmış bir klinik görüşme aracı olan Okul Çağı Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi- Şimdi ve Yaşam Boyu versiyonu uygulanmıştır. Bulgular: Çalışmamıza Gaziantep Üniversitesi Tıp Fakültesi Çocuk ve Ergen Psikiyatrisi Kliniğine başvuran ve TSSB tanısı almış 40 çocuk ve ergen dahil edildi. 13ʼü %32,5 erkek, 27ʼsi %67,5 kızdı. Yaş ortalaması 14,7 ± 2,87 olarak bulundu. 9 %22,5 hastada ebeveyn kaybı mevcuttu. 13 hastada sadece TSSB %32,5 tanısı vardı. Diğer hastalarda birden fazla tanı vardı. 12 %30 hasta daha önce psikiyatri kliniğine başvurmuş olmasına rağmen, diğer 28 %70 hasta daha önce başvuru yapmamıştı. Hastaların yaşadığı travma ile uzmanlık başvurusu arasındaki ortalama süre 3,97 ± 3,4 yıldı. 24 hastada %60 yalnızca bir travmatik olay yaşanmıştı, 16 hastada %40 birden fazla travmatik olay yaşanmıştı. Cinsel istismar n=19, %47,5 hâkim travma tipiydi.Sonuç: Bu küçük örneklemli çalışma psikiyatri kliniklerine başvuran TSSBʼli çocuk ve ergenlerin tanınması açısından önem arz etmektedir. Ancak yine de daha büyük örneklem sayılarına sahip çalışmalara ihtiyaç vardır

Sociodemographic and Diagnostic Characteristics of Post-Traumatic Stress Disorder Cases Presenting to a Child and Adolescent Psychiatry Department

Objective: Posttraumatic stress disorder PTSD is a debilitating psychiatric disorder that impacts many children and adolescents. The aim of this study was to determine the sociodemographic and diagnostic characteristics of PTSD patients presenting to a child and adolescent psychiatry clinic.Material and Methods: Children and adolescents who participated in this study were asked to fill a questionnaire about sociodemographic data and were asked questions about the traumatic event. In addition, they were evaluated with the Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime version K-SADS , a semi-structured interview for psychiatric diagnoses.Results: Forty children and adolescents presenting to the Gaziantep University School of Medicine’s Department of Child and Adolescent Psychiatry and diagnosed with PTSD were included in the study. There were 13 32.5% boys and 27 67.5% girls. The mean age was 14.7 ± 2.87 years. Nine 22.5% patients had lost a parent. Thirteen 32.5% patients had PTSD only whereas the rest had more than one psychiatric diagnosis. Twelve 30% patients had presented to a psychiatry clinic previously and 28 70% patients had not. The mean time between trauma and admission was 3.97 ± 3.4 years. A total of 24 patients 60% had only one traumatic event while 16 patients 40% had more than one traumatic event. Sexual abuse n=19, 47.5% was the dominant trauma type. Conclusion: This study had a small sample size but it ensures better understanding of the characteristics of children and adolescents with PTSD who present to psychiatry clinics. Studies with larger sample sizes are needed.
